经眶上锁孔入路手术摘除垂体瘤27例疗效分析

Operative technique of hypophyseoma through transobital keyhole approach

目的探讨经眶上锁孔入路手术摘除垂体瘤的可行性及临床应用价值。方法 2002年07月～2010年10月,对27例垂体瘤采取经眶上锁孔入路手术进行治疗,并对手术的安全性、疗效、并发症进行总结分析。结果肿瘤全切除24例,大部分切除3例,除2例并发暂时性尿崩症外,其余均无严重的神经功能损害。结论眶上锁孔入路能充分显露鞍区的结构,有利于暴露及切除向鞍上生长及浸润的肿瘤;且手术创伤小,术后并发症少,恢复快,不失为治疗垂体瘤的一种有效、安全的手术途径。

Objective To evaluate the effect of the transobital keyhole approach for resection of hypophyseoma.Methods Retrospective study was undertaken of 27 patients with hypophyseoma by transobital keyhole approach between 2002 and October 2010.Results Of 27 patients,24 had total resection,and 3 had subtotal resection.Nobody had severe nerves functional lesion during surgery.Surgical complications were found in 3 patients.Conclusion The new approach has its own specific advantage of small skin incision which does not damage the patients looks and short hospitalization.Keyhole approach is safe and effective to remove hypophyseoma.
