摘要
目的:了解高血压病人24h动态血压的控制状况。方法:以2010年1月-2011年3月在上海市嘉定区中医医院住院的、接受24h动态血压监测的106例高血压病人为观察对象,分为糖尿病组(n=34)和非糖尿病组(n=72),对监测结果进行回顾性分析。结果:病人的24h平均收缩压、日间平均收缩压、夜间平均收缩压,糖尿病组分别为(143±21.2)、(144±20.2)、(142±25.4)mmHg,非糖尿病组分别为(133±21.8)、(134±21.7)、(131±23.3)mmHg,糖尿病组显著高于非糖尿病组(P<0.05)。糖尿病组各项降压指标达标率均低于非糖尿病组;两组的收缩压和夜间血压下降比例达标率均低于本组的舒张压下降达标率。两组间抗高血压药物应用情况无显著性差异。结论:需加强对合并糖尿病高血压病人各时段收缩压和夜间血压的控制,重视动态血压监测对降压治疗的指导作用。
Objective:To understand control status of 24 h dynamic blood pressure in patients with hypertension. Methods: One hundred and six patients with hypertension who were monitored by 24 h dynamic blood pressure in Traditional Chinese Medical Hospital of Shanghai Jiading District from January 2010 to March 2011 were divided into diabetic group(n= 34) and non-diabetic group(n = 72) according to their disease status. The results were analyzed retrospectively. Results: Statistic data showed that 24 h mean systolic blood pressure, mean diurnal systolic blood pressure, and mean nocturnal systolic blood pressure in diabetic group were (143 + 21.2), (144 + 20. 2), (142+ 25.4) mmHg, respectively. And those data in non-diabetic group were (133+21.8),(134+21.7), (131+23.3) mmHg,respectively. The data in diabetic group were significantly higher than those in non-diabetic group(P〈0.05). The control rate of blood pressure in diabetic group was lower than that in non-diabetic group. The decreaed rates of systolic pressure and nocturnal blood pressure were lower than that of diastolic pressure between the same group. There was no significantly difference in the utilization of antihypertension drugs between two groups. Conclusion: Systolic blood pressure and nocturnal blood pressure of patients with hypertension and diabetes need more aggressive therapy, and more attention should be paid to monitoring dynamic blood pressure which could provide the guidance for the treatment of hypertension.
