临床药师在心血管内科的临床药学实践

Clinical pharmaceutical practice of pharmacists in Department of Cardiovasology

目的:介绍空军总医院的临床药师在心血管内科开展临床药学实践的模式。方法:以案例分析的形式,总结临床药师开展临床药学实践的经验。融入临床前,学习该科常见疾病的药物治疗方案和常用药物的作用特点;临床实践中,分析医嘱,并解决潜在的用药问题;以群体药动学为基础设计药物个体化给药方案;开展用药咨询,指导病人正确用药,回答医护人员咨询的临床药学问题。结果和结论:临床药师的用药建议有利于制定合理的给药方案。

Objective： To introduce the model of clinical pharmacy practice in Department of Cardiovasology, General Hos pital of Air Force of PLA. Methods： Experience of clinical pharmacy practice of clinical pharmacists was summarized by cases analysis. Clinical pharmacists should learn pharmaceutical therapeutic regimens of common diseases and characteristics of commonly used drugs. So that they can analyze prescriptions and solve latent problems, design the individualized drug administra tion on the basis of population pharmacokinetics, carry out medication consultation, guide patients to rational use of drugs and answer the clinical pharmaceutical problems consulted by medical staff. Results and Conclusion： The medication suggestion of clinical pharmacists is benefit of rational dosage regimen.
