摘要
目的通过对肠易激综合征(IBS)患者焦虑状态和或抑郁状态的筛检,了解IBS各亚型焦虑状态和抑郁状态的患病比例及与性别的相关性。方法随机选取100名经消化专科门诊确诊IBS并经HAD评分≥9分患者100例,根据患者症状分为腹泻型伴或不伴腹痛,便秘型伴或不伴腹痛四组,分别进行汉密尔顿焦虑(HAMA)及抑郁量表(HAMD)的测评。结果在经过HAD自评后的IBS患者,焦虑状态与抑郁状态的发生率分别为49%和29%,焦虑合并抑郁状态14%;便秘型伴腹痛组患者处于焦虑、抑郁状态均较其余组高,分别为35.7%和37.9%;便秘型伴有腹痛组的HAMA、HAMD评分较其余各组高,且具有明显差异;女性分别占焦虑和抑郁患病人数的57.1%和62.1%,均高于男性。HAMA得分在不同性别组具有明显差异性,而HAMD却无明显差异,女性的得分高于男性。结论 HAD是有效筛查IBS患者焦虑状态和抑郁状态的可靠工具;IBS患者各亚型中焦虑状态和抑郁状态的病比例不同;女性IBS焦虑和或抑郁发病率较男性为高。
Objective The status of anxiety and/or depression were screened in the patients of irritable bowel syndrome(IBS) to get the incidence rates of the subgroups and analyze the correlation of sex.Methods One hundred patients were selected randomly from the gastroenterology clinic,if they were diagnosed as IBS confirmly and the scores of HAD were more than or equal to 9.And then,patients were classified into four groups(diarrhea-type with or without gripes;Constipation-type with or without gripes) according to the symptoms.Every patients were evaluated by the HAMA and HAMD.Results The incidence rates of anxiety and depression of the included patients were 49% and 29% respectively.And 14% patients had the two syndroms simultaneously.The patients classified as constipation-type with gripes had higher percentage of anxiety(35.7%) and depression(37.9%) than other groups.Also,the scores of HAMA and HAMD were higher in this group with a significant difference.57.1% and 62.1% patients of anxiety and depression were females.From the analysis of sex,there was a statistic difference of HAMA score,and no similar result was obtained from HAMD score.But females had higher scores than males both in the evaluations of HAMA and HAMD.Conclusions HAD was a reliable tool to screen the status of anxiety and depression in the patients of IBS.Subgroups of IBS had different proportions of anxiety and depression.And females of IBS had higher incidences of anxiety and depression than males.
