格列美脲联合二甲双胍治疗2型糖尿病的成本-效果分析

Cost-effectiveness Analysis of Glimepiride Combined with Metformin in the Treatment of Type 2 Diabetes Mellitus

目的:比较格列美脲联合二甲双胍与单用格列美脲治疗2型糖尿病的成本-效果。方法:将我院198例2型糖尿病患者按就诊时间的先后次序分为A组(96例)和B组(102例)。A组格列美脲起始剂量1mg,qd,早餐服用;二甲双胍0.25g,tid,三餐前服用。B组格列美脲起始剂量2mg,qd,早餐服用。2组总疗程均为8周。观察治疗前后患者空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白、血脂、肾功能、体重指数,同时观察低血糖及其他不良反应发生情况,并进行成本-效果分析。结果:2组总有效率及不良反应发生率比较,差异无统计学意义(P>0.05)。分别以FPG和2hPG为诊断依据计算治疗8周后2组的成本-效果比(C/E)为2.43/2.43和3.83/3.76。结论:单用格列美脲治疗2型糖尿病与格列美脲联合二甲双胍治疗方案疗效相当,但联合治疗方案治疗2型糖尿病成本低于单用格列美脲。

OBJECTIVE： To compare cost- effectiveness of glimepiride combined with metformin in treatment of type 2 diabe- tes mellitus. METHODS： 198 cases of type 2 diabetes mellitus were divided into group A （96 cases）and group B （102 cases）. Group A was received glimepiride （1 mg, qd）combined with metformin （0.25 g, rid）and group B was received glimepiride （2 mg, qd）. To observe FPG, 2hPG, HbAlc, blood fat, renal function, BMI, and adverse effect and so on. RESULTS：Total effective rates and incidence rates of adverse effect of group A and group B were no significant difference. C/E of group A and B after 8 weeks were 2.43/2.43 and 3.83/3.76 based on FPG and 2hPG respectively. CONCLUSIONS： Therapeutic effect is no significant difference between glimepiride group and combination group. The cost of combination therapy is lower than that of glimepiride therapy.
