社区护理干预提高糖尿病患者遵医率和血糖控制的效果

Effects of Community Nursing Intervention for Improving the Compliance and Controlling the Blood Glucose of Patients with Diabetes

目的探讨社区护理干预提高糖尿病患者遵医率和血糖控制的效果。方法将70例2型糖尿病患者采用随机数字表法分为研究组和对照组各35例,两组患者在住院期间均接受系统的健康教育,出院后仅对研究组患者实施社区护理干预措施。出院后均随访1年,分别测定两组患者出院时及随访结束时的空腹和餐后2h血糖,同时比较两组患者随访期间的遵医率。结果两组患者均顺利完成随访。研究组患者经社区护理干预后,在定期复查、饮食控制、服药情况、自我监测和运动疗法等5个方面的遵医率分别为71.4%、80.0%、94.5%、80.0%及68.6%,高于对照组的45.7%、42.9%、68.6%、42.9%及34.3%,差异均有统计学意义(P<0.05);研究组患者空腹和餐后2h血糖较出院时均明显降低,亦低于同期对照组,差异有统计学意义(P<0.05)。结论社区护理干预可有效提高糖尿病患者的遵医率及血糖控制效果。

Objective To investigate the effect of community nursing intervention for improving the compliance and controlling the blood glucose of patients with diabetes. Methods Seventy patients with type 2 diabetes were randomly divided into study group and control group,with 35 cases in each group. The patients in both groups received systemic health education in the duration of hospitalization,and only the patients of the study group received community nursing intervention after discharge. All the patients were followed up 1 year after discharge. The fasting and postprandial 2h blood sugar were measured respectively at discharge from hospital and at the end of follow-up. Then the compliance rates were compared between the two groups during follow-up period. Resuits All patients completed the follow up successfully. The compliance rates in periodic review,alimentary control,overdose status,self-monitoring and exercise therapy of patients in study group alter the community nursing intervention（71.4% ,80.0% ,94.5%,80.0% and 68.6%, respectively） were significantly higher than those of the control group（45.7%,42. 9%,68.6%,42. 9% and 34. 3%,respectively）（P〈 0.05）. The fasting and postprandial 2h blood sugar values of the study group at the end of follow-up were not only significantly lower than those at discharge from hospital but also signilicantly lower than the control group（P〈0.05）. Conclusion Community nursing intervention can improve the compliance of diabetic patients and control the blood sugar effectively.
