无创正压通气治疗慢性阻塞性肺病合并呼吸衰竭临床效果观察

Clinical Efficacy of Noninvasive Positive Pressure Ventilation on Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

目的探讨无创正压通气治疗慢性阻塞性肺病合并呼吸衰竭的临床效果。方法选择慢性阻塞性肺病合并呼吸衰竭患者62例,以上患者被随机分为两组,观察组和对照组。观察组采用常规疗法,观察组在常规疗法基础上行无创正压通气治疗。观察两组治疗前和治疗24h后动脉血气改变情况。结果两组治疗后呼吸频率、心率、pH值、PaO2、PCO2分别与本组治疗前比较,差异有统计学意义(P<0.05);观察组治疗后呼吸频率、心率、pH值、PaO2、PCO2分别与对照组治疗后比较,差异有统计学意义(P<0.05)。结论无创正压通气能够显著改善慢性阻塞性肺病合并呼吸衰竭患者临床症状,治疗效果显著,值得借鉴。

Objective To study the clinical efficacy of noninvasive positive pressure ventilation on the patients with chronic obstructive pulmonary disease complicated with respiratory failure. Methods 62 cases with chronic obstructive pulmonary disease complicated with respiratory failure were selected and randomly divided into two groups, observation group and control group, routine treatment were performed in control group, and on the base of the routine treatment,noninvasive positive pressure ventilation was performed in observation group. The artery blood gas was detected before the treatment and 24 hours after treatment. Results Respiratory rate, heart rate, the value of pH, PaO2 and PCO2 after treatment compared with those before treatment in two groups respectively, there was statistical difference（P〈 0.05）; Respiratory rate, heart rate, the value of pH, PaO2 and PCO2 after treatment in observation group compared with those in control group after treatment, there was statistical difference（P〈0.05）. Conclusions Noninvasive positive pressure ventilation can improve the clinical symptoms of hronic obstructive pulmonary disease complicated with respiratory failure,and the therapy has significant efficacy.
