运动康复与骨折治疗一体化对肱骨远端骨折术后疗效的影响 被引量:13

Effect of Integration of Exercise Rehabilitation and Fracture Treatment on Postoperative Outcome in Patients with Distal Humerus Fractures

摘要 目的探讨运动康复与骨折治疗一体化对肱骨远端骨折术后疗效的影响。方法将2 762例肱骨远端骨折患者根据不同的康复治疗方法分为2组:一体化组1 401例和对照组1 361例。一体化组患者采用运动康复与骨折治疗一体化康复治疗,对照组患者按传统康复治疗方法。观察2组患者术后2周,1、3个月的临床疗效及术后并发症发生等情况。结果经过13~49(30.5±4.8)个月的随访,一体化组术后2周,1、3个月疗效优秀率均明显高于对照组,差异均有统计学意义(24.3%、46.2%、66.8%vs 14.3%、22.0%、39.0%,均P<0.05)。一体化组并发症共发生152例(10.8%),对照组并发症共发生242例(17.8%),2组患者并发症发生率比较差异有统计学意义(P<0.05)。结论运动康复与骨折治疗一体化治疗肱骨远端骨折可明显改善患者的预后,提高疗效,减少并发症发生率。 Objective To evaluate the effect of the integration of exercise rehabilitation and fracture treatment on postoperative outcome in patients with distal humerus fractures. Methods A total of 2 762 patients with distal humerus fractures were randomly assigned to receive either integrated exercise rehabilitation and fracture treatment (integration group, n = 1 401) or traditional rehabilitation (control group, n = 1 361). Postoperative complications were observed and curative effects were assessed 2 weeks, 1 month and 3 months after operation. Results Patients were followed-up for an average of (30.5±4.8) months (range 13 to 49 months). At 2 weeks, 1 month and 3 months after operation,the rates of excellent efficacy in integration group were significantly higher than those in control group (24.3% ,46.2% and 66.8% vs 14.3%,22.0% and 39.0 %, respectively, P〈0.05). Furthermore, the incidence of postoperative complications in integration group was significantly lower than that in control group (10.8% vs 17.8% ,P〈0. 05). Conclusion The integration of exercise rehabilitation and fracture treatment can significantly improve the prognosis,enhance therapeutic effect and reduce the incidence of postoperative complications in patients with distal humerus fractures.
