摘要
目的:为提高医院处方质量提供参考。方法:随机抽取我院处方点评工作开展前(2008年)、开展后第1年(2009年)及第2年(2010年)的第1季度门诊处方(每月2000张),以《医院处方点评管理规范(试行)》为标准,运用Excel软件处理数据,统计不合格、不合理处方,并对门诊药房同一时期所发生的药患纠纷进行统计。结果:不合格处方常见为药品剂量、规格书写不规范;不合理处方表现为遴选药品不适宜、药品剂型与给药途径不适宜等;与处方点评工作开展前相比,开展后的处方质量得到显著性提升((22.88±0.58)%vs.(14.85±0.43)%、(7.32±0.25)%,P<0.01),因处方质量欠缺所引发的药患纠纷也有显著性下降((19.33±1.15)%vs.(14.00±1.73)%、(6.67±1.53)%,P<0.01)。结论:处方点评具有提高处方质量与减少药患纠纷的双重作用,对提升医院及药师的整体形象、构建和谐的就医环境具有积极意义,应当长期开展下去。
OBJECTIVE: To provide reference for the improvement of prescription quality. METHODS: 2 000 prescriptions were randomly collected from the outpatient prescriptions every month of the first quarter in 2008, 2009 and 2010, when they were before and after the prescription evaluation carried out. Then the prescriptions were evaluated according to Hospital Management Practices Prescription Comments (trial), and all the unqualified and unreasonable ones were analyzed statistically using Excel soft- ware. In addition, the disputes between pharmacists and patients in outpatient pharmacy were analyzed statistically. RESULTS: Un- qualified prescriptions commonly were non-standard writing of drug dosage and specification; unreasonable prescriptions manifested as unsuitable drug selection, unreasonable drug dosage and route of administration, etc. The quality of prescriptions increased obviously ( (22.88 ± 0.58) % vs. ( 14.85 ± 0.43) %, (7.32 ± 0.25) %, P〈0.01 ) while the disputes caused by quality deficiency between pharmacists and patients decreased ((19.33 ± 1.15)% vs. (14.00 ± 1.73)% ,(6.67 ±1.53)% ,P〈0.01), compared to the ones before prescription evaluation carried out. CONCLUSIONS: Prescription evaluation not only improves the quality of prescription, but also reduces the disputes between pharmacists and patients, and it is important in enhancing the overall image of hospitals and pharma- cists and structuring harmonious environment for medical treatment. Therefore, it should adhere to practice prescription evaluation for further development.
