社区高血压健康管理志愿者管理新模式效果分析

Analysis on the Effect of the Management Mode of Healthy Volunteer in Community Hypertension

目的探讨以社区健康管理志愿者为主的、社区责任医生团队为指导的管理新模式,分析该模式对高血压患者随访管理所取得的效果。方法在洞桥社区卫生服务中心抽取所辖1个村,村内招募2名健康管理志愿者,经培训后上岗,对高血压患者进行健康管理,时间为1年的综合干预。结果管理新模式实施后,高血压患者管理有显著效果,服药率、血压控制率和规范管理率有明显的提高,分别达到89.5%、62.3%和86.5%,并且患者的生活方式得到了很大改善。结论采用社区健康管理志愿者管理新模式,既可以减轻社区责任医生的工作压力,又提高了高血压管理的效率,对目前农村社区卫生服务中心面临的人力资源不足是一条有效的途径。

OBJECTIVE To assess community health management of volunteer based community responsibility of the physician team guidance supplemented by a new management model for hypertension patients were follow-up management obtained actual effect. RESULTS The new mode of management of blood pressure in patients with mostly get effective control, patient medication rate, the blood pressure control rate, standardized mmlagement rate has been significantly improved, and the patient＇s lifestyle has been further improved. CONCLUSION To enable community health management of hypertension patients in community volunteers to management not only can reduce the community responsibility of doctors working pressure and reach the expected goal, on the current rural community health service center facing the lack of human resources is an effective way.
