重症患者NBP与IBP监测的相关性研究 被引量:7

The correlation between non-invasive blood pressure and invasive blood pressure monitoring in critical ills

摘要 目的观察重症患者不同病理状态下有创血压测量(IBP)与无创血压测量(NBP)间的差异性及其相关性。方法将100例重症患者根据入科时NBP检测收缩压水平分为三组,即A组(SBP<90 mmHg)、B组(90mmHg≤SBP<140 mmHg)、C组(SBP≥140 mmHg),同时进行桡动脉IBP和肱动脉NBP监测,所得数据进行对比分析。结果①A组,有创收缩压及舒张压均相应显著低于无创检测值(P<0.05);②B组I,BP和NBP间比较无统计学意义(P>0.05);③C组,有创收缩压值显著高于无创收缩压值,而有创舒张压的值显著低于无创舒张压值(P<0.05);④两种检测方法间相关性分析显示,A组患者收缩压下两种方法的相关系数为0.595、0.492,C组患者相应为0.633、0.650,而B组患者达0.8160、.837。结论不同病理状态下IBP与NBP存在一定差异,对低血压及高血压的重症患者应积极进行IBP监测,以利于及时准确地了解血压和组织灌注,以提高抢救成功率。 Objective To investigate the difference and correlation between invasive blood pressure(IBP) and non-invasive blood pressure(NBP) in critical ills.Methods 100 critical ills were divided into three groups according to measurement systolic blood pressure(SBP) of earlier admission to ICU: group A,SBP〈90 mmHg;group B,90 mmHg≤SBP〈140 mmHg;group C,SBP≥140 mmHg.The invasive blood pressure on radial artery and non-invasive blood pressure on brachial artery were monitored every four hours.The data of three groups were analyzed.Results In group A,the measurements of IBP were significantly lower than the measurements of NBP(P〈0.05);In group B,there were no significant difference between IBP and NBP(P〉0.05);In group C,the measurements of ISBP was significantly higher than that of NSBP,and the measurements of IDBP was significantly lower than that of NDBP(P〈0.05);The Spearman Rank Correlation Coefficients between IBP and NBP of three groups were 0.595 and 0.492 in group A,0.816 and 0.837 in group B and 0.633 and 0.650 in group C,respectively.Conclusion There were some differences between IBP and NBP in different pathological conditions.Under conditions,such as shock or hypertension,the invasive blood pressure should be actively monitored in order to recognize the real blood pressure and tissue perfusion,improve the rate of resuscitation.
