中西医结合治疗精神分裂症疗效观察

Therapeutic Effect of Traditional Chinese Medicine Combined with Western Medicine for Schizophrenia:A Report of 113 Cases

【目的】观察中西医结合治疗精神分裂症的临床疗效。【方法】采取随机单盲法将113例精神分裂症患者随机分为中西医结合治疗组(中西药组)64例和西药组49例。西药组每人每日给予服用效价相当于氯丙嗪400 mg的抗精神病药物,中西药组在西药组治疗的基础上,结合中医辨证治疗,两组均以8周为1个疗程。治疗前后进行简明精神病量表(BPRS)评定,并观察两组临床疗效和毒副反应情况。【结果】(1)总有效率中西药组为95.31%,西药组为93.88%。两组临床总有效率比较,差异无统计学意义(P>0.05),说明两组的临床疗效相仿。(2)BPRS评定结果:中西药组在治疗1～4周减分显著快于西药组,差异有统计学意义(P<0.05或P<0.01),说明中西药组在治疗的显效时间即控制急性期症状方面显著优于西药组;而第8周末两组评分比较,差异无统计学意义(P>0.05)。(3)中西药组出现锥体外系副作用者(10/64)明显低于西药组(12/49),两组比较差异有统计学意义(P<0.01),说明中西医结合治疗能有效减少药物副反应。【结论】与单纯西药治疗比较,运用中西药结合治疗精神分裂症,在控制患者急性期症状及减少药物毒副作用方面疗效显著。

Objective To observe the therapeutic effect of traditional Chinese medicine combined with western medicine for schizophrenia.Methods A randomized single-blind trial was carried out in 113 schizophrenia patients.The patients were divided into combination group（N=64） and western medicine group（N=49）.The two groups were given antipsychotic which had equivalent titer of 400mg of chloropromazine each day,and the combination group was additionally given herbal medicine based on syndrome differentiation.Eight weeks constituted one treatment course.The score based on brief psychiatric reacting scale（BPRS） was evaluated before and after treatment,and the therapeutic effect as well as toxic and side reaction was observed after treatment.Results（1） The total effective rate was 95.31% in the combination group and 93.88% in the western medicine group,the difference being insignificant（P〉0.05）.（2） The decrease of BPRS score was obvious in the combination group after treatment for 1～4 week（s）（P〈0.05 or P〈0.01 compared with that in the western medicine group）,indicating that combination group had shorter time on making an effect and relieving acute stage symptoms.However,the difference of BPRS score was insignificant between the two groups at the end of week 8（P〉0.05）.（3） The incidence of extrapyramidal side effects in the combination group（10/64）was lower than that in the western medicine group（12/49）,the difference being significant（P〈0.01）.Conclusion Traditional Chinese medicine combined with western medicine for schizophrenia has better effect on relieving acute stage symptoms and reducing toxic and side effects than western medicine alone.
