摘要
目的探讨多囊卵巢综合征(PCOS)患者血清Ghrehin水平与胰岛素抵抗的关系。方法选择我院2010年3月到2011年3月期间60例PCOS患者,根据体重指数(BMI)随机分为两组:肥胖组(A组,BMI≥25kg/m2)和非肥胖组(B组,BMI<25kg/m2),每组30例。并选取60例健康受试者作为对照组,其中肥胖组(C组,BMI≥25kg/m2)和非肥胖组(D组,BMI<25kg/m2)各30例。采集以上四组患者空腹静脉血,测定空腹血糖(FBG)、空腹胰岛素(FINS)水平,计算胰岛素抵抗指数(HOMA-IR);采用酶联免疫吸附法(ELISA)测定血清Ghrelin水平。结果①与健康受试者比较,PCOS患者的FINS、HOMA-IR均明显增高(P<0.05),血清Ghrelin明显降低(P<0.05),FBG无明显差异(P>0.05);②在PCOS患者和健康受试者中:与非肥胖组比较,肥胖组的FINS、HOMA-IR均明显增高(P<0.05),血清Ghrelin明显降低(P<0.05),FBG无明显差异(P>0.05);③PCOS患者血清Ghrelin水平与BMI呈显著负相关(r=-0.59,P=0.006),与WHR呈显著负相关(r=-0.24,P=0.021),与HOMA-IR呈显著负相关(r=-0.40,P=0.003)。结论 PCOS患者Ghrelin水平降低,并以PCOS肥胖组降低尤为明显;PCOS患者Ghrelin水平与胰岛素抵抗相关。
Objective To explore the relationship between serum ghrelin and insulin in women polycystic ovary syndrome (PCOS). Methods Sixty patients diagnosised with PCOS and another sixty health volunteers were also selected. PCOS patients or the volunteers were classified into two groups respectively: Obese group (BMI i〉25 kg/m2) and non-obese group (BMI 〈25 kg/m2). There were 30 cases in each group. Serum Ghrelin level of the four groups were detected by Enzyme Linked Immunossassy Assay (ELISA) method. Fasting blood glucose (FBG), fasting insulin (FINS) were also detected. Then the HOMA-IR was computed by FBG and FINS. Results ①The FINS and HOMA-IR of PCOS patients were significantly higher than the volunteers (P 〈0.05), Serum Ghrelin level of PCOS patients were significantly lower than the volunteers (P〈0.05).②In both the PCOS patients and volunteers: the FINS and HOMA-IR of obese groups were significantly higher than non-obese groups (P〈0.05), Serum Ghrelin level of obese groups were significantly lower than non-obese groups (P 〈0.05); ③Serum Ghrelin levels of the women with PCOS were correlated negatively with BMI (r =-0.59, P=0.006), with WHR (r =-0.24, P =0.021), with HOMA-IR (r =-0.40, P =0.003). Conclusion Serum Ghrelin levels of women with PCOS decrease in contrast to health women, particularly in obese women with PCOS. The decrease is correlated with HOMA-IR.
