2型糖尿病左室舒张功能及颈动脉损害的相关研究

Study on the Correlation Between Left Ventricle Diastolic Function and Carotid Artery Damage in Patients with Type 2 Diabetes Mellitus

摘要 [目的]探讨2型糖尿病(T2DM)对左室舒张功能及颈动脉损害的影响.[方法]随机选取T2DM患者136例(T2DM组),健康对照组90例,均采用标准方法检测血压、测量身高(m)、体重(kg);检测血糖、血脂等生化指标.采用标准方法测定E峰与A峰比值(E/A),并测量颈动脉内膜中层厚度(IMT).[结果]T2DM组E峰值及E/A比值显著低于对照组,[(53.43±12.78) cm/s VS (72.82±14.73) cm/s;(0.76±0.32) VS (1.24±0.39)](P〈0.05),IMT显著高于对照组[(0.89±0.25)mm VS (0.53±0.18)mm](P〈0.05),颈动脉斑块发生率、斑块积分高于对照组,差异有统计学意义(P〈0.05).以E/A比值为因变量,自变量包括年龄、BMI、TG、TC、FPG、2hPG、HOMA-IR、LDL.年龄、2hPG,HOMA-IR为心室舒张功能的独立危险因素(β值分别为0.187、0.005、0.172,P值分别为0.034、0.043、0.012).[结论]糖尿病患者更易发生左室舒张功能及颈动脉损害. [Objective] To explore the effect of type 2 diabetes mellitus on left ventricle diastolic function and carotid artery damage. [Methods]Totally 136 patients with type 2 diabetes mellitus(DM group) and 90 healthy subjects were selected randomly. Blood pressure, height, weight, blood glucose, lipids, the ratio of E peak to A peak(E/A) and carotid intima-media thickness(IMT) of all subjects were measured by using standard method. [Results] The E peak and E/A ratio in DM group were lower than those in control group[(53.43 ±12.78)cm/s vs (72.82±14.73)cm/s, (0.76±0.32)cm/svs (1.24±0.39)cm/s](P〈0.05). IMTinDM group was higher than that in control group [(0.89±0.25)mm vs (0. 53±0.18)mini( P 〈0.05). The incidence of carotid atherosclerotic plaque and plaque score in DM group were higher than those in control group, and there were significant differences between two groups( P 〈0.05). The ratio of E/A was the dependent variable, and the age, BMI, TG, TC, FPG, 2hPG, HOMA-IR and LDL were the independent variables. The age, 2hPG and HOMA-IR were the independent risk factors of left ventricle diastolic function(β= 0. 187, 0. 005,0. 172, P =0. 034,0. 043, 0. 012). [Conclusion] The patients with diabetes mellitus are apt to have the damage of left ventricle diastolic function and carotid artery.
