肺泡灌洗治疗高血压脑出血并发肺感染的临床疗效观察

Observation on the Clinical Efficacy of Bronchoalveolar Lavage for the Treatment of Hypertensive Cerebral Hemorrhage Complicated with Pulmonary Infection

[目的]探讨支气管肺泡灌洗治疗高血压脑出血并发肺感染的临床效果.[方法]选择本院GCS 4~8分重症高血压脑出血病并肺部感染已行气管切开患者60例,随机分成试验组与对照组,每组各30例,试验组加行支气管肺泡灌洗,对照组未予支气管肺泡灌洗,其他治疗措施两组均基本相同.观察比较两组的临床治疗效果.[结果]试验组有26例肺部感染控制,控制时间（8.2 ±2.1）d,对照组20例肺部感染控制,控制时间（14.3 ±2.9）d,试验组肺部感染控制率明显高于对照组,控制感染平均时间明显短于对照组,且两组相比差异有显著性（均P〈0.05）.[结论]支气管肺泡灌洗能有效控制高血压脑出血肺部感染,缩短住院时间,提高患者生存率和生活质量.

[Objective] To explore the clinical efficacy of bronchoalveolar lavage for the treatment of hypertensive cerebral hemorrhage complicated with pulmonary infection. [Methods] Totally 60 patients with GCS4-8 severe hypertensive cerebral hemorrhage complicated with lung infection undergoing tracheotomy were randomly divided into the exper imental group and the control group with 30 cases in each group. The experimental group was additionally treated with bronchoalveolar lavage and the control group was not treated with bronchoalveolar lavage. The other therapeutic measures in two groups were basically similar. The clinical efficacy was compared between two groups. [Results] In the experimental group, 26 cases had the control of pulmonary infection, and the control time was （8.2 ±2.1）d. In the control group, 20 cases had the control of pulmonary infection, and the control time was （14.3±2.9）d. The control rate of pulmonary infection in experimental group was higher than that in control group, and the mean time of pulmonary infection control in experimental group was shorter than that in control group, and there were significant differences between two groups（all P 〈0.05）. [Conclusion] Bronchoalveolar lavage can effectively control pulmonary infection in hypertensive cerebral hemorrhage, shorten the hospitalization time and improve the survival rate and quality of life.
