癌抗原125检测在维持性血液透析患者透析充分性评估中的临床意义

Significance of Dectecting the Concentration of Cancer Antigen 125 in Dislysis Sufficiency of Maintenance Hemodialysis Patients

目的探讨癌抗原125(CA125)检测在维持性血液透析患者透析充分性评估中的临床意义。方法血液透析前、透析结束前30min动脉端采血标本检测尿素氮BUN、CA125浓度,计算单室模型尿素清除指数spKt/V值、评估透析充分性,并比较spKt/V值与CA125水平的相关性。结果 32例新入维持性血液透析患者透析2月后,13例透析充分,spKt/V值≥1.3,CA125为(11.68±4.35)KU/L;19例透析充分性不足,spKt/V值均<1.3,CA125为(89.34±13.67)KU/L;二者CA125平均水平差别有统计学意义(P<0.05)。19例透析不充分患者调整透析方案后,8例spKt/V值≥1.3,CA125为(14.28±6.84)KU/L;11例患者spKt/V值仍均<1.3,CA125为(78.61±10.22)KU/L;二者CA125水平差别有统计学意义(P<0.05)。结论维持性血液透析患者CA125水平与透析充分性、尤其spKt/V值具有一定相关性。CA125简单易测,检测该指标可部分反映血液透析患者透析充分性。

Objective To investigate the significance of the concentration of cancer antigen 125 in dialysis sufficiency of maintenance hemodialysis patients.Methods The serum urea nitrogen and CA125 were measured before hemodialysis beginning and before 30 minute hemodialysis ending.Calculate spKt/V and evaluate dialysis sufficiency.Compare correlation between spKt/V and CA125.Results 32 new patients were enrolled.After two months,spKt/V of 13 patients≥1.3,the average concentration of CA125 were（11.68±4.35）kU/L.spKt/V of 19 patients1.3,the average concentration of CA125 were 89.34±13.67kU/L.Difference of CA125 concentration between two groups patients was significant（P〈0.05）.After four months,spKt/V of 8 patients among 19 patients≥1.3,the average concentration of CA125 were（14.28±6.84）kU/L,but spKt/V of the other 11 patients 1.3,the average concentration of CA125 were（78.61±10.22）kU/L,Difference of CA125 concentration between two subgroups patients was significant（P〈0.05）.Conclusions The concentration of venous CA125 of maintenance hemodialysis patients was correlate with dialysis sufficiency,especially with spKt/V.Dectecting concentration of CA125 was used to investigate dialysis sufficiency in maintenance hemodialysis patients.
