自体骨与异体骨腰椎椎间融合临床疗效的比较

Comparison of clinical outcome of autogeneous bone and allograft bone in lumbar spine fusion

目的比较自体骨及异体骨进行腰椎间融合的临床治疗效果。方法根据椎间融合材料的不同,将268例分为自体骨组和异体骨组,回访术后植骨融合情况及临床症状改善情况。结果两组术后临床症状改善明显。统计学分析表明两组在手术时间、出血量、输血量、住院时间及最终融合率方面无明显差别。自体骨组在椎间隙高度丢失率、平均融合速度与异体骨组有显著性差异。术后并发症自体骨组明显低于异体骨组(P<0.05)。结论自体骨与异体骨在腰椎融合术中疗效相近,但自体骨组术后并发症更少。

Objective To evaluate clinical outcome of autogeneous bone and allograft bone in lumbar spine fusion.Methods Two hundred and sixty eight patients were divided into autogeneous bone group and allograft group.Clinical data of two groups were reviewed after operation.Results Clinical symptoms were relieved significantly in two groups after operation.Statistical analysis showed that there were no significant differences between two groups in the operation time,bleeding volume,volume of blood transfusion,hospitalization time and the final fusion.There were significant differences in intervertebral space height loss rate and average fusion rate between two groups.Complications in autologous bone group was significantly lower than that in the allograft group(P <0.05).Conclusion Autogeneous bone and allograft bone in the lumbar fusion are similar,but autogeneous bone fusion has less complications than allograft bone fusion in a long-term follow-up.
