极外侧型腰椎间盘突出症的诊治

Experience of diagnosis and treatment of extreme lateral prolapse of lumbar intervertebral disc

目的 总结极外侧型腰椎间盘突出症手术治疗的体会。方法 极外侧型腰椎间盘突出症患者16例,根据病理类型采用后正中入路椎板扩大开窗术和横突间入路单纯椎间盘切除术。结果 术前VAS评分为(7.63±0.64)分,与术后(1.46±0.45)分相比较,差异有统计学意义(P<0.01)。末次随访Macnab评定:优43.75%(7/16),良31.25%(5/16),可12.50%(2/16)。结论 手术治疗极外侧型腰椎间盘突出症可获得良好近期效果,手术方式应依据突出椎间盘组织占位、病理类型及椎管内病变而定。

Objective To summary of diagnosis and treatment of extreme lateral prolapse of lumbar intervertebral disc.Methods The type of intervertebral disc was 16 cases of patients,according to the pathological types used in the middle of the road after the lamina expanded the window and the art at the road between pure discectomy.Results VAS score of preoperative was（7.63±0.64）compared with（1.46±0.45）after surgery,significant was difference（P 〈 0.01）.Macnab assessment：excellent 43.75%,good in 31.25%,general 12.50%.Conclusion Surgical treatment of extreme lateral prolapse of lumbar intervertebral disc can get a good short-term effect.Surgical procedure decision should be based on herniated organization placeholder,pathological type and spinal lesions.
