摘要 目的:探讨联合检测血清肿瘤标志物癌胚抗原(carcinoembryonic antigen,CEA)、神经元特异性烯醇化酶(neurons specificenolase,NSE)、细胞角蛋白19片段(CYFRA21-1)在肺癌诊断中的应用价值。方法:对74例肺癌组,46例健康对照组,33例肺部良性疾病组,采用电化学发光法进行CEA、NSE、CYFRA21-1检测并对比分析。结果:肺癌组的三项肿瘤标志物水平均高于肺良性疾病和健康对照组。CEA、NSE、CYFRA21-1分别在腺癌、小细胞癌、鳞癌中表达水平最高,其阳性率分别为82.6%、81.3%、68.6%,与其他单项肿瘤标志物比较差异有统计学意义(P<0.05),且三项联合检测对肺癌诊断敏感性明显高于单项标志物。结论:血清CEA、NSE和CYFRA2I-1的联合检测有助于临床对肺癌的诊断,并对组织分型有较高的参考价值。 Objective:To study the application value of joint test of serum tumor markers carcinoembryonic antigen(CEA),neurons specific enolase(NSE) and cytokeratin 19 fragment(CYFRA21-1) in the diagnosis of lung cancer.Methods:The CEA,NSE and CYFRA21-1 detections in 74 cases of lung cancer,46 cases of healthy controls and 33 cases of benign lung disease were performed by the electrochemical luminescence method and the results were compared.Results:The three tumor markers in the lung cancer group were higher than those in the benign lung disease group and healthy controls.CEA,NSE and CYFRA21-1 were highest in gland cancer,small cell carcinoma and squamous cell carcinoma,their positive rates were 82.6%,81.3% and 68.6% respectively,showing significant difference compared with other single tumor markers(P0.05).And the sensitivity of 3-item combination detection to lung cancer diagnosis was obviously higher than that of single marker.Conclusion:The joint test of serum CEA,NSE and CYFRA2I-1 is helpful to the clinical diagnosis of lung cancer with higher reference value to the histologic classification.
