腓肠神经皮瓣加持续冲洗治疗跟骨开放骨折

Treatment of open calcaneal fractures with sural nerves skin flap and continuous irrigation

目的探讨开放性跟骨骨折的治疗方法。方法 2003～2010年共收治跟骨开放骨折伴皮肤缺损19例,采用腓肠神经营养血管皮瓣修复,负压封闭、持续冲洗。结果 19例患者创面均愈合,浅表感染2例,骨折延迟愈合3例,全部患者获得6～12个月(平均10个月)的随访,术后皮瓣全部成活,足跟部皮瓣外形良好,能够负重。结论腓肠神经皮瓣移植并负压封闭、持续冲洗治疗跟骨开放骨折,在有效地稳定骨折的同时,有效地封闭创面,缩短创面修复时间,促进骨折愈合,减少并发症,其治疗方法操作简单,成功率高和疗效好。

Objective To explore the treatment of open calcaneal fractures. Methods All of 19 cases of open calcaneal fractures with skin defects from 2003 to 2010, were treated with suralnerves skin flap, Vacuum sealing and continuous irrigation. Results All the cases were followed up for 6 to12 months （averaging 10 months）. All of the grafts survived,superficial infection in 2 cases and 3 case delayed union.Their posterior heels regained fair appearance,could walk with weight- hear ing. Conclusion Sural nereve skin flap with vacuum sealing and continuous irrigation is a simple and effective treatment method for open calcaneal fractures,because it can prove effective and rapid fixation,seal wounds,shorten healing time of wounds,facture union and prevent complications.
