舒芬太尼复合不同麻醉镇静药用于门诊无痛人流的效果比较

Comparison of effect using sufentanil combined with different drugs for painless induced abortion in outpatients

目的：比较舒芬太尼复合依托咪酯乳剂与丙泊酚用于门诊无痛人流的效果及优缺点。方法：选择ASA I~Ⅱ级门诊人工流产患者130例,随机分为两组,每组65例。A组舒芬太尼0.15ug/kg＋依托咪酯乳剂0.2mg/kg,B组舒芬太尼0.15μg/kg＋丙泊酚2mg/kg,静脉给药后观察术中血压、心率、SpO2、呼吸暂停、注射痛、清醒时间、肌紧张、术后恶心呕吐情况及患者满意率。结果：两组间注射痛、清醒时间、患者满意率无差别（P〉0.05）。B组血压、心率下降明显,呼吸抑制发生率高于A组。A组肌阵挛、术后恶心呕吐发生率高于B组（P〈0.05）。结论：舒芬太尼复合依托咪酯乳剂与丙泊酚均能为无痛人流术提供理想满意的麻醉效果,舒芬太尼复合依托咪酯乳剂对呼吸循环影响轻微,舒芬太尼复合丙泊酚肌紧张、术后恶心呕吐发生率低。

Objective： To compare the effect and adverse reaction using sufentanil combined etomidate and propofol emulsion for painless induced abortion in outpatient.Methods： ASA I ~ Ⅱ grade 130 patients of abortion clinics,were randomly divided into A and B groups（65 cases each group）.Sufentanil（1.5ug.kg-1） and etomidate（0.2mg.kg-1） was used in group A,sufentanil（1.5ug.kg-1） and propofol（2mg.kg-1） was used in group B,observed after intravenous administration of intraoperative blood pressure,heart rate,SpO2,respiration suspension,injection pain,recovery time,muscle tension,postoperative nausea and vomiting and patient satisfaction rate.Results：The difference of injection pain,recovery time and patient satisfaction rate was not significant in two groups（P〉0.05）.Blood pressure,heart rate decreased and respiratory depression was higher in the group B（P〈0.05）.The myoclonus and the incidence of postoperative nausea and vomiting was higher in the group A（P〈0.05）.Conclusion： sufentanil combined etomidate and propofol emulsion as a painless abortion could provide the ideal satisfactory anesthesia,etomidate and sufentanil combined minimal impact on respiration and circulation.Sufentanil complex propofol has minimal impact on muscle tension,and the incidence of postoperative nausea and vomiting was lower.
