腹腔镜胆囊切除术482例并发症分析

The Analysis of the 482 Examples of the Complications of Laparoscopic Cholecystectomy

目的:探讨如何尽量避免腹腔镜手术的并发症。方法:回顾分析和总结腹腔镜胆囊切除手术482例临床资料。结果:出现胆总管高位横断后第一肝门处左右肝管完全离断缺损1例,胆总管高位横断后残留0.5cm左右肝管1例,因解剖不清晰开腹6例,钛夹夹闭不全致胆漏1例,迷走胆管漏1例,术中胆囊破裂冲洗未吸尽致局限性腹膜炎1例,腹腔炎性渗液致局限性腹膜炎1例。结论:牢固树立责任意识,克服麻痹思想,才能避免手术的损伤。

Objective：discuss how to avoid laparoscopic surgery complication.Methods： Reviewing and summarizing the clinical data of the 482 Laparoscopic Cholecystectomy.Result： high transaction of common bile duct to the left and right hepatic duct completely defect（1 case）,high transaction of common bile duct to the left and right hepatic duct high residual 0.5CM（1 case）,laparotomy（6 cases）,bile leakage caused by clipping lax after operation（1 case）,vagus bile leakage（1 case）,limitations of peritonitis caused by not exhausting the bile that flows into the abdominal cavity as the gallbladder rupture in the operation（I case）,abdominal infection caused by the limitations of peritonitis（1 case）.Conclusion： Surgical trauma can be avoided by strengthening the sense of responsibility,studying step by step and overcoming carelessness.
