摘要
目的通过监测新生儿脐血及出生后2h外周血血糖,了解正常新生儿血糖的变化,及时发现正常新生儿低血糖,并进行干预。方法选择220例孕足月、无妊娠合并症的不同分娩方式的新生儿,分别检查脐血血糖和产后2h血糖。结果分娩方式不同,新生儿低血糖发生率不同,择期剖宫产明显高于平产及急诊剖宫产。正常新生儿脐血低血糖发生率4.1%,2h低血糖发生率7.2%,脐血低血糖与2h低血糖发生率呈正相关性。结论不能忽视正常新生儿低血糖的发生率,可以用新生儿脐血血糖监测方法替代外周血血糖监测,其有无创、方便、快捷特点。
Objective To explore the clinical value of monitoring the glucose level in the cord blood of neonates born by different modes of delivery, with an attempt to provide clues for the early identification and intervention of neonatal neonatal hypoglycemia. Methods Totally 220 otherwise healthy full-term newborns which were born by different modes of delivery were enrolled in this study. Their cord blood glucose levels and blood glucose levels 2h after delivery were determined. Results The incidence of neonatal hypoglycemia was significantly higher among neonates born by elective Caesarean section (7.5% in the cord blood and 11.25% in the peripheral blood 2 h after birth) than those by normal delivery (both 3.75% in the cord blood and in the peripheral blood 2 hours after birth) or emergency cesarean section (both 3.33% in the cord blood and in the peripheral blood 2 h after birth). Among these otherwise healthy neonates, the rate of low glucose level in cord blood was 4.1%, which was positively correlated with the rate of low glucose level in the peripheral blood was 7.2%. Conclusion The occurrence of hypoglycemia among healthy neonates should not be neglected. As a non-invasive, simple, and fast technique, monitoring of the glucose level in the cord blood may be a good alternative for peripheral blood glucose test.
