超脉冲CO_2点阵激光与优化脉冲光治疗皮肤瘢痕色素沉着的临床疗效观察与比较

Therapeutic Effect on Hyperpigmentation of Dermal Scar by Optimal Pulse Technology and Ultrapulse CO_2 Fractional Laser:an Observation and Comparison

目的比较优化脉冲光(optimal pulse technology,OPT)与超脉冲CO2点阵激光治疗皮肤瘢痕色素沉着的疗效及安全性方法皮肤瘢痕伴色素沉着患者206例,随机分为两组。OPT组109例,采用OPT治疗仪治疗。点阵激光组97例,采用超脉冲CO2点阵激光治疗。治疗后随访1年,进行临床疗效评估比较两组疗效。结果大部分患者治疗后,色素沉着均逐渐消褪。OPT组患者中23例治愈,49例显效,32例有效,5例无效,总有效率95.4%。点阵激光治疗组12例治愈,32例显效,43例有效,10例无效,总有效率89.7%。其中表浅瘢痕色素沉着疗效较显著。结论优化脉冲技术治疗皮肤瘢痕色素沉着为无创治疗,效果明显,复发率较低,对表浅瘢痕的色素沉着效果较好,但对增生性瘢痕的色素沉着疗效欠佳。CO2点阵激光为微创治疗,疗程短,且对增生性瘢痕色素沉着亦有一定效果。

Objective To observe and compare the clinical efficacy and operation safety between the optimal pulse technology （OPT） and ultra- pulse CO2 fractional laser in the treatment of hyperpigmentation of dermal scar. Methods A total of 206 cases with hyperpigmentation of dermal scar were divided into two groups randomly, the treatment group was treated by OPT and the control group treated by ultrapulse CO2 fractional laser. The result and side effect of the treatment were recor- ded. Results After one year follow-up the hyperpigmentation among 206 cases was nearly disappeared. The curative effect rate of the treat- ment goup was 95.4% vs 89.7% in the control goup .Conclusions OPT is effective, safe, atraumatic and valuable for the treatment of hyperpigmentation with less complications, and su- perior for the hyperpigmentation of superficial scar. CO2 fractional laser is minitraumatic with short period of treatment, it is also effec- tive in treating hyperpigmentation of hyperplastic scar.
