胃癌患者S100A4蛋白高水平表达与淋巴结转移及胃癌不良预后的相关性 被引量:2

High-level expression of S100A4 is correlated with lymph node metastasis and poor prognosis in patients with gastric cancer

摘要 目的 探讨S100A4在胃癌组织中的表达及其与胃癌浸润、转移及预后的关系.方法 应用免疫组织化SP法检测S100A4在436例胃癌组织及92例非肿瘤胃黏膜组织中的表达情况.结果 胃癌组织中S100A4表达显著高于癌旁非肿瘤胃黏膜组织,436例胃癌组织中有261例S100A4蛋白表达阳性,187例高表达,S100A4蛋白的阳性表达与患者的年龄、肿瘤部位、肿瘤大小、浸润深度、脉管侵犯、淋巴结转移、远处转移及TNM分期有关,而与患者性别、分化程度、组织学类型无关.I、II和III期S100A4高表达胃癌患者5年生存率显著低于S100A4低表达者,IV期胃癌患者的5年生存率与S100A4表达无关,多变量分析表明S100A4表达、浸润深度、淋巴结和远处转移和TNM分期是胃癌患者独立的预后因素.结论 胃癌组织S100A4表达与淋巴结和远处转移及预后显著相关,S100A4可能成为一个预测肿瘤进展和预后的分子标记物. Objective To investigate the expression of S100A4 in gastric cancer and its association with tumor development, progression and metastasis. Methods Immunohistochemistry was employed to analyze S100A4 expression in tissue specimens of 436 cases with gastric cancer. Results S100A4 protein was detected in 261 of 436 cases of human gastric cancer, and highly expressed in 187 cases. Positive expression of S100A4 was correlated with age, tumor location, size, depth of invasion, lymph node and distant metastases, Lauren classification, regional lymph node stage, and TNM stage; but not correlated with sex, differentiation, or histological classification. In stage I, Ⅱ, and Ⅲ tumors, the 5-year survival rate in patients with high expression of S100A4 was significantly lower than that in patients with low expression. In stage IV tumors, expression ofS100A4 was not correlate with the 5-year survival rate. Multivariate analysis suggested that depth of invasion, lymph node and distant metas- tases, TNM stage and upregulation of S100A4 were independent prognostic indicators for the disease. Conclusion Expression of S100A4 in gastric cancer is significantly associated with lymph node and distant metastases, and poor prognosis. S100A4 may be a useful marker to predict development, progression, and metastasis of gastric cancer.
