母婴血清胆红素与肝酶的相互关系及意义

摘要 目的 探讨妊娠母亲和新生儿的血清胆红素与肝酶的相互关系及意义。方法 同时测定118组配对妊娠母亲、脐血、新生儿和42例对照组成人的血清总胆红素(BilT)、结合胆红素(BilD)和肝酶(ALT、AST、GGT、LDL),并将母亲和新生儿的血清BilT、BilD与肝酶结果作相关分析。结果 ①血清BilT、BilD含量高低依次为新生儿>脐血>妊娠母亲、对照组(P<005);妊娠母亲与对照组的血清BilT、BilD差异无显著意义(P>005)。②血清ALT、AST、LD—L含量高低为母亲>对照组(P<005);GGT含量高低为新生儿、脐血>母亲、对照组(P<005),新生儿与脐血之间、母亲与对照组之间GGT含量差异无显著意义(P>005);血清AST、LDL含量高低为新生儿>母亲、对照组(P<005),新生儿与对照组的血清ALT差异无显著意义(P>005)。③母亲的血清胆红素水平与肝酶含量相关(P<001),新生儿的血清胆红素含量与肝酶无相关关系(P>005)。结论 妊娠母亲与对照组成人的血清胆红素含量无统计学差异,新生儿出生后血清胆红素水平逐渐增高。母亲的血清BilT、BilD含量与肝酶的变化一致,新生儿的血清BilT、BilD的变化不受肝酶含量的影响。 Objective To search into the relation between serum bilirubin and hepatozymases in pregnant women and their newborns and assess the significance therein Methods The total bilirubin(Bil T),conjugated bilirubin(Bil D) and hepatozymases(AST,ALT,GGT and LD L) in the serum of 118 paired pregnant women(PW),cord blood(CB),newborns(NB) and 42 control adults(CA) were measured,and the relations between serum bilirubin and hepatozymases in PW and their NB were analyzed simultaneously.Results ① The rank order of Bil T and Bil D levels(L 1)was:L 1 NB >L 1 CB >L 1 PW and L 1 CA ( P <0.05);there was no difference between L 1 PW and L 1 CA .②The rank order of ALT,AST and LD L levels(L 2) was:L 2 PW >L 2 CA ( P < 0.05 ).The rank order of GGT levels (L 3) was:L 3 NB and L 3 CB >L 3 PW and L 3 CA ( P <0.05); there was no difference between L 3 NB and L 3 CB ,and no difference between L 3 NB and L 3 CB ( P >0.05). The rank order of AST and LD L levels (L 4) was:L 4 NB >L 4 CA and L 4( P <0.05), but in terms of ALT, there was no difference between L NB and L CB ( P >0.05).③There was a correlation between the levels of serum bilirubin and hepatozymases in PW( P <0.01), but such correlation was not noted in NB( P >0.05).Conlusion No significant difference in the serum bilirubin levels was observed between the pregnant women and the control adults. The serum bilirubin levels of the newborns gradually increased after birth. In the pregnant women, the serum Bil T and Bil D levels changed with the levels of the serum hepatozymases; in the newborns, the changes of hepatozymases levels had no effect on the levels of serum bilirubin.
