摘要 目的评价中国[未包括香港、澳门特别行政区和台湾地区(港、澳、台)]2010年麻疹监测系统(Measles Surveillance System,MSS)的运转状况,为加强麻疹监测提供参考。方法分析31个省(自治区、直辖市,下同)2010年通过MSS上报的疑似麻疹病例(Measles-1ike Illnesses,MLIs)个案数据库,评价MSS的主要运转指标。结果 2010年全国共报告MLIs56766例(包括现住址为外籍和港、澳、台病例40例,下同),其中38181例为麻疹(包括现住址为外籍和港、澳、台病例22例),排除麻疹18529例,另有56例MLIs的病例分类待定。2010年排除麻疹病例报告发病率为1.39/10万,全国36.7%的县(区、市、旗)>1/10万。56766例MLIs中,51736例(91.14%)报告了个案流行病学调查信息,50449例(88.87%)在报告后48h内进行了个案调查(25个省>80%)。53688例为散发病例,血标本采集率76.63%(14个省>80%)。2010年共采集45224份血标本,其中38233份(84.54%)检测结果报告日期距采血日期≤7d(20个省>80%)。2010年共报告316起麻疹爆发疫情,其中300起(94.94%)为实验室确诊麻疹爆发,127起(40.19%)采集了病原学标本。结论中国MSS运转质量逐步提高,主要监测指标达到监测方案要求,但各省MSS运转不均衡,需进一步提高MSS运转质量,以适应消除麻疹需要。 Objective To evaluate the running status of measles surveillance system (MSS)of China (Hong Kong Special Administrative Region, Macao Special Administrative Region, Taiwan region were excluded) in 2010. Methods We reviewed the case investigation data of measles-like Illnesses (MLIs) reported by 31 provinces (municipals, autonomous regions)through MSS in 2010. Performance indicators of the MSS were analyzed. Results Totally 56,766 MLIs were reported through MSS in 2010. Of which 38,181 were classified as laboratory or clinically confirmed measles cases, 18,529 were discarded measles cases, and 56 were unclassified. In 2010, reported incidence of discarded measles case was 1.39 per 100,000 population, and 36.72% of all counties exceeded 1 per 100,000 population national wide. Proportion of MLIs investigated within 48 hours of report was 88.87 %, proportion of MLIs with adequate serum specimens was 76.63%, and percentage of serum specimen with testing results reported within 7 days of collection was 84.54%. Of the 316 measles outbreaks, 94.94% were laboratory-confirmed, and 40.19% collected specimen for virus detection. Conclusions The steadily increased performance indicators are clear steps of improved MSS in China. However, further efforts in measles surveillance are needed to achieve the goal of measles elimination.
