摘要 目的应用动态血糖监测观察血糖波动对于糖尿病肾病患者尿白蛋白排泄的影响。方法 78例2型糖尿病肾病患者,采用动态血糖监测系统(CGMS)对其进行连续24 h血糖监测,测定其平均血糖水平、血糖波动水平,并进行相关分析。结果患者平均血糖水平(Glu-Ave)、平均血糖波动水平(Glu-SD)及平均尿白蛋白排泄率(UAER)分别为(8.05±1.60)mmol/L、(2.01±1.30)mmol/L及(153.10±67.80)μg/min。Pearson相关分析显示,UAER与病程、Glu-Ave、Glu-SD呈正相关(r分别为0.250、0.527、0.543)(P<0.05)。结论糖尿病病程、Glu-Ave及Glu-SD是糖尿病肾病的重要危险因素。积极控制血糖,减少血糖波动对防止糖尿病肾病的发生和发展有一定的帮助。 Objective To observe effect of blood glycemic fluctuation on albumin excretion rate(UAER) in diabetic Nephropathy.Methods Methods:Seventy-eight cases of type 2 diabetes mellitus were were monitored for 24 hours to detect and analyze average glycemic level(Glu-ave),Glycemic fluctuation(Glu-SD)and UAER.Results The Glu-Ave,Glu-SD and mean UAER were(8.05±1.60)mmol/L,(2.01±1.30)mmol/L and(153.10±67.80)μg/min respectively.Pearson analysis showed that UAER was positively correlated with the course of the disease,Glu-Ave and Glu-SD(r value were 0.250,0.527,0.543 respectively)(P0.05).Conclusion The course of diabetes,Glu-Ave and Glu-SD were important risk factors in diabetic nephropathy.Energetic blood glucose control and diminishing glycemic fluctuation were helpful to the prevention of diabetic nephropathy.
