摘要
:〔目的〕总结结肠造口术后并发症的发病情况及探讨其防治措施。〔方法〕回顾性分析自1988年～1998年浙江省肿瘤医院654例结直肠癌行结肠造口术后其造口并发症的发病率及发病原因。〔结果〕结肠造口术后并发造口狭窄的发生率为17%,术后肠梗阻26%,造口周围皮肤炎31%,腹部切口感染57%,无造口坏死、回缩及脱垂发生。总的术后并发症的发生率为131%。其中17例肠梗阻中13例由肠粘连所致,4例由肠内疝所致。切口感染和造口周围皮肤炎常与术前肠道准备不充分,及术后护理不仔细有关。〔结论〕充分的术前肠道准备,经腹膜外隧道造口,采取平坦形造口及术后加强护理常能有效地降低造口术后并发症的发生率。
urpose〕To investigate the etiologic factors complications following colostomy and to explore the proceduce for prevention complications.〔Methods〕Six hundred and fifty five cases with colorectal cancers undergone colostomies in our hospital from 1988 to 1998 were analyzed retrospectively.〔Results〕All of 654 cases with colostomy,the incidence of postoperative stomal stenosis was 1.7 percent;intestinal obstruction,2.6%;peristomal dermatitis,3.1%;wound infection 5.7%.No stomal ischemic necrosis,no retraction or prolapse were found. The cumulative complication rate of colostomy was 13.1 percent. Thirteen of 17 cases with intestinal obstruction caused adhesions,and 4 cases caused by internal herniation around the bowel.Peristomal dermatitis and wound infection often caused by poor preoperative bowel preparation and poor postoperative care.〔Conclusion〕Sufficient peroperative bowel preparation,extraperitoneal colostomy and extensive postoperative stomal care could decrease complication rate of colostomies effectively.
