安徽绩溪县晚期血吸虫病患者经济负担与影响因素

Investigation on the financial burden and influence factors for the advanced schistosomiasis patients in Jixi county,Anhui province

目的探讨安徽绩溪县晚期血吸虫病(晚血)患者医疗救治的家庭经济负担状况及其相关影响因素,为制定医疗救治政策提供科学依据。方法设计晚期血吸虫病患者的经济负担与影响因素个案调查表,由2名经过培训的医师进行逐项调查,调查内容包括患者基本信息、近三年其它疾患治疗和经济负担、晚血治疗直接费用和间接费用及救助情况、家庭经济背景情况等。结果 91例晚血病人,平均年龄为71.43±6.60岁;92.3%病人有2次以上晚血治疗史;59例伴有心血管疾病、胆囊炎等并发症,并发有两种以上疾病者39例。91例病人晚血医疗费用共计84 8 669.1元,人均9 326.03元;59例并发有其它慢性疾病者,有52例还因其它疾病进行过住院治疗,共计住院医疗费用451350元,人均8 679.81元。住院医疗费用由个人承担434550元,人均个人承担8520元,占总费用的96.28%。经主成分回归分析,对患者经济负担的影响主要有文化程度、家庭人口数、临床类型、并发症和夹杂症、主要成员的情况、基本生活保障和医疗保障等因素。结论晚血病人均为高龄老人,没有稳定的经济收入来源,很多伴发多种严重慢性疾病。每年的疾病治疗费用负担相当沉重,需要政府的医疗救治帮助,才能安度晚年。

Objective To study the financial burden and influence factors for the advanced schistosomiasis patients in Jixi county, and provide scientific basis for formulating medical treatment policy on advanced schistosomiasis. Methods To design investigation table, and the patient basic information, the treatment and financial burden in recent 3 years, the direct and indirect cost for the treatment of advanced schistosomiasis, the family economic condition, and other factors were inquired by two trained physicians. Results 91 cases of advanced schistosomiasis were investigated. The average age was 71.43 ± 6.60 years old. 92.3% of patients have more than 2 times treatment for advanced schistosomiasis. 59 cases had the complication of cardiovascular diseases and cholecystitis, and 39 cases had more than two kinds of complications. The total cost for the treatment of the 91 advanced schistomiasis was 84 8 669.1Yuan with the average cost of 9326.03 Yuan. Of the 91 cases, 59 had other chronic illnesses and 52 cases had hospital treatment for other diseases with the total cost of 45I 350 Yuan, and 96.28% of cost was undertook by the patients themselves. The principal component regression analysis showed the influence factors to the financial burden of advanced schistosomiasis were cultural degree, population of family, clinical type, complication, condition of main member of family, insurance for basic living and medical treatment. Conclusion Advanced schistosomiasis patients are elderly people in Jixi county, they have not stable economic income sources, and most of them have other serious chronic diseases. The finance burden for the medical treatment was quite heavy every year, and government should provide medical treatment help for their happy life in the old age.
