摘要
目的 探讨喉返神经修复手术治疗声带麻痹的效果及损伤的修复时机对预后的影响。 方法 采用了不同术式及不同时机施行神经修复手术治疗声带麻痹19例(单侧14例,双侧5例),其中即刻手术6例、延期手术13例,喉返神经直接吻合5例、膈神经与喉返神经远侧断端吻合同时颈袢胸骨舌骨肌支与喉内收肌支吻合1例、颈袢主支与喉返神经内收肌支吻合8例、颈袢胸骨甲状肌蒂填植入左环杓后肌5例。 结果 19例均有不同神经再支配,且即刻手术1例术后恢复最好,5例良好;延期手术13例中5例良好,6例改善,2例较差。 结论 即刻手术较延期手术效果好,延期手术12个月以上效果较差,喉返神经修复及移植术治疗声带麻痹,最晚勿超过1年。
Objective To investigate the time of reinnervation and prognosis of laryngeal paralysis after recurrent laryngeal nerve injuries Method Nineteen cases with laryngeal paralysis were immediately (6 cases) and delayed (13 cases) operated by direct recurrent laryngeal nerve suture and selective reinnervation of laryngeal muscles. Results After 6 months follow up, the effect was excellent in 1 case who was immediately operated by selective reinnervation of the abductor and adductor muscles of the larynx, good in 16 cases, and poor 2 cases who were delayed operated over 12 months Conclusion It was the last time not over 1 year to nerve repair or reinnervation The earlier reinnervation was performed, the better prognosis was
