摘要 目的 分析准分子激光原位角膜磨镶术 (LASIK)对高度近视的疗效。方法  2 30例 (4 6 0眼 )行LASIK手术的高度近视患者随访 1a ,根据术前屈光度分为 3组 ,对各组术后裸眼视力及其影响因素、术后剩余屈光度及并发症进行分析。结果 术后 40 0眼 (86 96 % )达到预期的最佳矫正视力。Ⅰ、Ⅱ、Ⅲ组达到最佳矫正视力分别为 92 6 2 %、87 45 %、77 11% ,切削区中心偏离瞳孔中心的距离 >0 5mm对术后裸眼视力有影响 ;术中多区域切削使达到最佳视力的百分率下降 (72 11% ) ;术后 3月屈光度趋于稳定 ,实际矫正度在预期矫正度± 1 0 0D内的百分率为 85 32 %。结论 LASIK术治疗高度近视疗效可靠 ,手术预测性及稳定性强。 Objective To analyse and evaluate the therapeutic effects of laser in situ keratomileusis(LASIK)for correction of high degree myopia. Methods 230 patients with high degree myopia (460 eyes)treated by LASIK were followed up.They were divided into three groups:the first group to 6.00~10.00 D,122 eyes;the second group to 10.25~20.00 D,255 eyes;the third group to 10.25~29.00 D ,83 eyes.Uncorrected visual acuity and influencing factors,residual refraction and complications after operation were analysed in a one-year-follow up study. Results After LASIK,400 eyes (86.96%)attained best corrected visual acuity,92.62%attained their best corrected visual acuity in the fitst group,87.45%in the second group and 77.11%in the third group.The uncorrected visual acuity was affected greatly when decentration of ablated center area was larger than 0.5 mm from pupillary center.The tapered transition zone technique decreased the percentage of the best corrected visual acuity attained postoperactivetly(72.11%).The refraction was stable and 85.32% were within ±1.00 D in three months. Conclusions LASIK for treatment of high degree myopia is an effective and safe refractive surgery and the accuracy and predictability are good.Decentration of ablatin from pupillary center and tapered transition zone affect the therapeutic effects.
