Expression of Cyclind1,CDK4 and P16 in Osteosarcoma and its Clinical Significance

摘要 目的探讨CyclinD1、CDK4和P16在骨肿瘤中的表达及临床意义。方法应用免疫组化和原位杂交方法检测105例骨肿瘤组织中的CyclinD1、CDK4和P16的表达,并对其中85例骨肉瘤主要临床资料、病理分级及临床相关参数进行比较,用χ2检验进行统计学处理。结果骨肉瘤中CyclinD1、CDK4蛋白及mRNA阳性表达明均显高于骨软骨瘤(P<0.01);骨肉瘤临床分期中Ⅱb期、Ⅲ期均高于Ⅰ期、Ⅱa期(P<0.01);随着病理分级恶性程度增高而显著增加(P<0.01);且在软组织浸润和转移组中明显高于非浸润和非转移组(P<0.01)。P16蛋白及mRNA阳性表达骨软骨瘤明显高于骨肉瘤(P<0.01);骨肉瘤的临床分期中Ⅱb期、Ⅲ期均低于Ⅰ期、Ⅱa期(P<0.01);随着病理分级恶性程度增高而显著降低(P<0.01);软组织浸润和转移明显低于非浸润和非转移组(P<0.01)。结论 CyclinD1、CDK4、P16的蛋白及mRNA在骨肉瘤中不同程度异常表达与肿瘤的临床分期、病理分级和浸润、转移有一定关系。提示CyclinD1、CDK4和P16蛋白及其mRNA高表达在骨肉瘤的发生、发展和浸润转移过程中发挥重要作用。 Objective To investigate the expression of cyclin D1,CDK4 and P16 in bone tumors and its clinical significance.Methods Immunohistochemistry and in situ hybridization were used to detect the expression of cyclin D1,CDK4 and P16 in 105 cases of bone tumor tissue;and the main clinical features,histological grade and clinical parameters in 85 cases of osteosarcoma were compared and analyzed with Chi-square test.Results The positive rate of cyclin D1 and CDK4 protein and mRNA expression in osteosarcoma was significantly higher than that of osteochondroma(P0.01),and was higher in stage Ⅱb and Ⅲ period of osteosarcoma as compared to stage Ⅰ and Ⅱa(P0.01),and increased with histological grade in the degree of malignancy(P0.01).The rate in the groups with soft tissue invasion and metastasis was significantly higher than that in group without invasive and metastasis group(P0.01).As for P16,the protein and mRNA expression in osteochondroma was significantly higher than that in osteosarcoma(P0.01);the positive rate in stage Ⅱb and Ⅲ were lower than that in stage Ⅰ and Ⅱa(P0.01),and decreased with the degree of malignant tumor grade(P0.01);The rate in the group with soft tissue invasion and metastasis was significantly lower than that in group without invasive and metastasis(P0.01).Conclusion The expression of cyclin D1,CDK4 and P16 protein and mRNA in osteosarcoma correlate positively with the tumor clinical stage,pathological grade,invasion and metastasis,and suggest that cyclin D1 and CDK4 and P16 play an important role in the process of the invasion and metastasis of osteosarcoma.
