欣普贝生与催产素在足月妊娠促宫颈成熟及引产的对比观察

Comparison of Effects of Dinoprostone Vaginal Suppositories and Oxytocin on Induced Labor in Term Pregnancy

目的比较欣普贝生与催产素用于足月妊娠促宫颈成熟及引产的效果。方法将2008年1月-2011年6月北京煤炭总医院160例有引产指征的足月妊娠孕妇随机分成两组,研究组以欣普贝生阴道后穹窿置药,对照组小剂量催产素静脉点滴引产,比较两组宫颈Bishop评分、引产成功率、分娩情况以及对产妇、胎儿和新生儿的影响。结果研究组促宫颈成熟的有效率93.75%、引产成功率88.75%、平均临产时间为(10.99±3.44)h、剖宫产率15%。对照组促宫颈成熟的有效率62.5%、引产成功率53.75%、平均临产时间为(38.84±13.95)h、剖宫产率51.25%。两组比较,差异有统计学意义(P<0.001)。两组产妇、胎儿、新生儿影响无显著性差别。结论欣普贝生用于足月妊娠促宫颈成熟和引产明显优于催产素,而且用药安全方便,值得在临床上推广应用。

Objective To compare the effects of dinoprostone vaginal suppositories and oxytocin on cervical ripening and induced labor in term pregnancy.Methods One hundred and sixty term pregnant women who required induction of labor from Jan.2008 to Jun.2011 in our hospital were randomly divided into observation group（80 cases） and control group（80 cases）;The dinoprostone suppositories was put into the posterior fornix of pregnant women in observation group,and small dose oxytocin was dripped in vein of pregnant women in control group;the Bishop scores,success rate of induced labor,modes of delivery and influence of mater and neonate were compared between the two groups.Results The efficacy of cervical ripening,the rate of successful labor induction,the average time from medication to in labor and the rate of cesarean section in observation group were 93.75%,88.75%,（10.99±3.44）h and 15%.The efficacy of cervical ripening,the rate of successful labor induction,the average time from medication to in labor and the rate of cesarean section in control group were 62.5%,53.75%,（38.84±13.95）h and 51.25%.It was significantly different between the two groups（P0.001）.While,there was no significant difference in outcome of mater and neonate between the two groups.Conclusion Dinoprostone vaginal suppositories is superior to oxytocin on cervical ripening and labor induction in term pregnancy.It＇s safe and convenient.It deserves to promote the application in clinic.
