多重耐药的铜绿假单胞菌Ⅰ类整合子与其多重耐药基因的相关性分析 被引量:10

摘要 目的调查本院多重耐药铜绿假单胞菌耐药基因的存在状况,Ⅰ类整合子的分布情况以及其与多重耐药基因相关性分析。方法采用K-B法测定临床分离的铜绿假单胞菌对15种药物的耐药性,筛选出35株多重耐药菌株,采用聚合酶链反应(PCR)检测β-内酰胺酶编码基因、OPrD2基因、氨基糖苷类修饰基因和Ⅰ类整合子基因。结果 35株多重耐药铜绿假单胞菌中β-内酰胺酶编码基因CTX-M-1、TEM、SHV的检出率分别为51.4%、31.4%、14.0%;OPrD2基因的阳性率为28.6%;氨基糖苷类修饰基因aac(6′)-Ⅱ、aac(6′)-Ⅰ、ant(3″)-Ⅰ和ant(3″)-Ⅱ的阳性率分别为20%、2.86%、14.3%和17.1%;Ⅰ类整合子阳性的菌株中,CTX-M-1、TEM、SHV、aac(6′)-Ⅱ和ant(3″)-Ⅰ的检出率为45.4%、9.1%、27.3%、36.4%和27.3%,未检出aac(6″)-Ⅰ和ant(2″)-Ⅰ基因。结论本院多重耐药铜绿假单胞菌存在多种耐药基因,常见的有β-内酰胺酶编码基因、OPrD2基因和氨基糖苷类基因。Ⅰ类整合子广泛存在于多重耐药的铜绿假单胞菌中,并携带多种耐药基因参与形成铜绿假单胞菌的多重耐药。 Objective To investigate the existing status of drug-resistant gene,distribution ofⅠintegrons and its correlation with drug-resistant gene in multidrug-resistant Pseudomonas aeruginosa.Methods The resistance of clinically isolated Pseudomonas aeruginosa to 15 kinds of different antibiotics was tested by K-B method,and 35 strains of multidrug-resistant bacteria were screened out.Beta-lactamase genes,OPrD2,aminoglycoside-modifying enzyme genes and integrons Ⅰgenes were detected by polymerasechain reaction(PCR).Results Among the 35 strains of multidrug-resistant Pseudomonas aeruginosa,the detection rates of beta-lactamase genes CTX-M-1,TEM and SHV were 51.4%,31.4% and 14.0% respectively.10 strains(28.6%) lost OPrD2 gene.The detection rates of aminoglycoside-modifying enzyme genes aac(6′)-Ⅱ,aac(6′)-Ⅰ,ant(3″)-Ⅰand ant(3″)-Ⅱ were 20%,2.86%,14.3% and 17.1% respectively.Eleven of the 35 isolates(35.2%)contained Ⅰintegron,and the detection rates of CTX-M-1,TEM,SHV,aac(6′)-Ⅱand ant(3″)-Ⅰwere 45.4%,9.1%,27.3%,36.4% and 27.3% respectively in positive integrons Ⅰstrains,and none of the aac(6″)-Ⅰand ant(2″)-Ⅰgenes.Conclusion Multidrug-resistant Pseudomonas aeruginosa in our hospital carries multidrug resistant genes,resistance to beta-lactamase,OPrD2 and aminoglycoside-modifying enzyme genes.Integrons Ⅰ is widely spreaded in multidrug-resistant pseudomonas aeruginosa,and carries multidrug-restitant genes to produce multidrug-resistance.
