小儿喉罩拔除时机与呼气末二氧化碳分压的相关性 被引量:7

摘要 目的:观察喉罩全麻自主呼吸恢复后,于不同呼气末二氧化碳分压(PETCO2)值拔除喉罩时患儿的气道及其他不良反应,探讨安全拔除喉罩时的PETCO2值。方法:纳入120例患儿,随机分为3组:40组,于PETCO240mmHg时拔除喉罩;45组,于PETCO245mmHg时拔除喉罩;50组,于PETCO250mmHg时拔除喉罩。每组各40例患儿。记录拔除喉罩前和拔除喉罩后即刻的血压和心率。同时观察拔除喉罩即刻及其后10min之内患儿是否有下颌松弛情况,及有无呛咳、咬管、屏气、喉痉挛等气道反应表现;有无低氧血症(SpO2<90%)。结果:50组下颌松弛的发生率明显高于40组与45组(P<0.05)。40组患儿呛咳发生率(9.09%)明显高于45组和50组(均为0%,P<0.05)。40组及45组患儿咬管、屏气及体动发生率分别明显高于50组(P<0.05)。3组间喉痉挛发生率和低氧血症发生率均无明显差异(P>0.05)。结论:小儿喉罩全麻后,当PETCO2为50mmHg时拔除喉罩呛咳、咬管、屏气、体动、喉痉挛及低氧血症等并发症的发生率较低,能较平稳地拔除喉罩;且拔除喉罩后仅有少数患儿出现轻度的气道不通畅情况,轻偏头部或轻托下颌可以纠正。
