Miles'手术中腹膜外间隙放置补片对预防造瘘口旁疝的疗效分析

Clinical effect of placement of a mesh in preperitoneal space during Miles' operation in the prevention of parastomal hernia

目的探讨腹膜外间隙放置补片对减少造瘘口旁疝的作用。方法选取我院2003年1月至2006年9月腹会阴直肠癌根治术病例85例,年龄25～60岁,分成两组,观察组行乙状结肠单口造瘘时在腹膜前间隙放置补片,共42例,其中女性19例,男性23例,对照组为单纯乙状结肠单口造瘘组,共43例,其中女性22例,男性21例。结果观察组无出现造瘘口旁疝病例,对照组中13例发生造瘘口旁疝,两组差异有统计学意义,两组在造瘘口相关的其他并发症上差别无统计学意义。结论在严格的无菌操作下,Miles`术中腹膜外间隙放置补片预防造瘘口旁疝是可行的。

Objective To investigate the clinical effect of placement of a mesh in the preperitoneal space during Miles＇ operation in the prevention of parastomal hernia.Methods 85 patients of 25~60 years old received Miles＇operation from Jan.2003 to Sep.2006 were selected.The patients were divided into two groups.The study group（n=42）.including 19 females and 23 males,received placement of a mesh in the preperitoneal space,while the control group（n=43）,including 22 females and 21 males,received no placement of a mesh in the preperitonel space.Results No case of parastomal hernia was found in the study group,while 13 cases were found in the control group.The two groups showed no statistically significant difference in other complications.Conclusion With strict aseptic technique,the placement of a mesh in the preperitoneal space during Miles＇ operation is practicable to prevent parastomal hernia.
