选择性阴茎背神经分支切断术治疗原发性早泄

Selective resection of dorsal penis nerve branches for primary premature ejaculation

目的:探讨选择性阴茎背神经分支切断术治疗原发性早泄的手术指征、方法及疗效。方法:距冠状沟上1.5cm处依次切开皮肤、肉膜、筋膜,充分显露背神经分支,保留1～2支,其余均予以切断、残端电灼。结果:对1316例原发性早泄患者术后随访1～3.5年,射精潜伏期达5min以上918例,3～5min 250例,症状无明显改善148例,有效率88.7%。结论:本术式创伤小、安全、疗效满意,是治疗原发性早泄的一种选择手段。

Objectives： To investigate the operation indications,methods and therapeutic effects of selective resection of dorsal penis nerve branches in treatment of primary premature ejaculation（PPE）.Methods： From coronary sulcus on 1.5 cm,we successively cut open skin,sarcolemma and fascia,fully exposed nervus dorsalis branches and kept 1~2 teams,the others all be cut off and fulgerized.Results： 1316 of them are followed up for 1~3.5 years,the ejaculation latency was significantly prolonged（above 5 minutes） in 918,improved（3~5 minutes） in 250 and not improved in 148 patients,with a total effective rate of 88.7%.Conclusion： This procedure of small trauma,safety and efficacy satisfaction,is a choice of primary premature ejaculation.
