分级分区管理模式在急诊输液安全管理中的应用与评价

Application and evaluation of hierarchical partition management mode in emergency transfusion safety management

目的:对急诊输液患者通过采用分级分区护理模式,以提高输液安全性,减少护患矛盾,提升患者的满意率。方法:从2010年起,对每一位输液患者进行安全评估,由医生或护士根据患者的病情、生命体征指标和使用药物的情况进行分级:患者的生命体征不稳定、使用某些需要密切观察病情的特殊药物的患者、输注血液制品的患者划分为一级患者,其他则为二级患者,并划分为红色高危区域和蓝色普通区域,对两个区域患者实施不同级别的输液管理。结果:此模式实施后,患者输液意外情况发生率较前下降,患者对护士输液技术、服务满意度等提高,而且护士对此模式的认同率和执行率也有所提高(P<0.05)。结论:采取分级分区的护理模式,可以有效减少因输液管理不慎引起的意外和隐患的产生,提升患者满意度,有效地保证输液护理安全。

Objective： To explore the effect of hierarchical partition management mode for the transfusion patients in emergency room, to improve transfusion safety, to reduce the transfusion accidents or hidden dangers, to decrease nurse-patient conflicts, and to improve patients＇ satisfaction. Methods：We assessed every infusion patient for safety from 2010 and classified them by a doctor or nurse according to patients＇ condition, vital signs and drug usage. Patients with unstable vital signs, using certain drugs which need close observation of patients＇ conditions, or infusion of blood products were divided into First- Grade patients, named red high-risk area, and other patients were divided into Second- Grade patients, named blue common ares. We managed these two areas with different levels of transfusion management. Results： The implementation of this model could decrease the transfusion accident rate significantly, improved patients satisfaction on nurses＇ infusion skills and service, increased nurses recognition rate and execution rate to the mode （P〈0.05）. Conclusion： This hierarchical partition management mode can effectively reduce the transfusion accidents and hidden dangers caused by management carelessness, enhance patient satisfaction, ensure infusion nursing safety.
