后内侧小切口辅助下单钢板治疗SchatzkerV、VI型胫骨平台骨折 被引量:5

摘要 目的探讨后内侧小切口辅助下单钢板治疗SchatzkerV、VI型胫骨平台骨折的方法与临床疗效。方法对32例SchatzkerV、VI型胫骨平台骨折患者行后内侧小切口辅助下复位并外侧有限切开单钢板植骨内固定术。胫骨骨折按Schatzker分型,V型22例,Ⅵ型10例;左侧18例,右侧14例。术后嘱患者抬高患肢,切口处弹力绷带适当加压包扎,以减少关节及切口肿胀。未合并韧带损伤者不用任何外固定,患者术后48~72h做膝关节被动活动,第3天开始膝关节主动活动,通常7~10d膝关节屈曲达到90°,不负重功能练习6~8周,8周后扶拐部分负重行走,12周后摄x线片示骨折愈合后可负重行走。结果32例患者均获得随访,随访时间6~24(16.62±2.42)个月。术后骨折愈合时间4~8(5.48±1.57)个月。无骨不连、关节面塌陷、切口感染及皮肤坏死等并发症。手术疗效评价采用Rasmussen胫骨骨折关节功能评分标准,优19例、良10例、可3例,优良率90.6%(29/32)。结论后内侧小切口辅助下单钢板治疗SehatzkerV、VI型胫骨平台骨折能降低软组织损伤的并发症,单钢板联合空心螺钉固定牢靠、稳定,有利于患者早期功能锻炼,促进膝关节功能恢复,是微创治疗Sehatzker V、VI型胫骨平台骨折的一种有效方法。
