三种治疗RockwoodⅢ-Ⅴ型肩锁关节脱位方法的对比研究 被引量:3

摘要 目的通过对三种治疗RockwoodⅢ-Ⅴ型肩锁关节脱位的方法的对比研究,找到治疗该脱位的最佳方法。方法将54例新鲜RockwoodⅢ-Ⅴ型肩锁关节脱位患者按照随机、均衡原则分为甲乙丙三组,甲组19例采用AO锁骨钩钢板固定、乙组17例采用喙突间加压螺钉固定、丙组18例复位后采用双Endobutton系统固定。术后对患者随访10~16个月,依据Karlsson标准回顾性分析比较治疗效果差异。结果双Endobutton系统治疗组患者肩关节功能评定优良率为94.44%,显著优于AO锁骨钩钢板组58.82%及喙突间加压螺钉组的47.37%,差异有统计学意义(χ2=24.24,P<0.05)。结论双Endobutton系统在治疗RockwoodⅢ-Ⅴ型肩锁关节脱位疗效上优于其他两种治疗方法。 Objective To compare the effect of the three conventional methods which used to remedy Rockwood III - V type acromioclavieular joint dislocation. Methods A total of 54 fresh cases suffered Rockwood III - V type aeromioclavicular joint dislocation were divided into three groups in accordance with random and balance principles. Group A (19 cases) were treated by AO clavicular hook plate fixation; group B (17 cases) were treated by compression screw fixation between the coracoid and group C (18 cases ) using dual Endobutton system reset fixed. After the surgery, followed up the patients in 10 to 16 months and evaluated the therapeutic effect according to Karlsson standard. Results The assessment of shoulder function of group C was with an excellent rate of 94.44%, significantly better than that of the group A (58.82%) and B (47.37%) (χ2= 24.24, P 〈0.05). Conclusions The therapeutic effect of double Endobutton system in the treatment of Rockwood III - V type on the acromioclavicular joint dislocation is better than the other two treatment methods.
