上海市白鹤社区实施糖尿病信息化管理的研究

目的:探讨上海市白鹤社区实施糖尿病信息化管理模式的成效,为社区糖尿病信息化管理提供依据。方法:以政府为主导成立糖尿病信息化管理中心,建立基于政务网的信息化管理网络,积极探索家庭医生制服务模式开展糖尿病专项管理。结果:通过管理糖尿病患者的糖尿病知识知晓率由69.91%上升到82.27%,血糖监测率由58.3%上升到78.2%,血糖达标率由42.36%上升到57.82%,糖化血红蛋白达标率由34.27%上升到59.88%。结论:建立社区糖尿病信息化管理中心是提高社区糖尿病患者疾病管理的有效途径,家庭医生制服务模式在农村地区具有可推广性。

Objective： This study is carried out to understand the effect of Computerized Management of Diabetes Mellitus in Baihe Town,and to provide evidence for computerized management of diabetes mellitus in communities. Methods： We established a government dominated diabetes mellitus computerized management center,and built a computerized management net work,positively exploring a mellitus specialized family doctor managing mode. Results： Through this management,patients＇awareness rate of diabetes mellitus rose from 69. 91% to 82. 27%, and glycaemia monitoring rate was raised from 58. 3% to 78. 2%,while success rate of gly- caemia and glycolated hemoglobin rose from 42. 3% to 57. 82% and 34. 27% to 59. 88% respectively. Conclusion： To establish diabetes mellitus computerized management centers is an effective way to improve management of diabetes mellitus patients in communities,and the family doctor servingmode is worthy to be spread in rural areas.
