通腑逐瘀汤治疗新生儿胃肠功能衰竭的临床观察

Clinical observation on treatment with Tongfu Zhuyu Decoction for gastrointestinal functional failure in infants

目的：观察通腑逐瘀汤配合西医治疗新生儿胃肠功能衰竭的疗效。方法：１０８例患儿随机分为治疗组（５４例）和对照组（５４ 例）。２ 组均采用西医治疗（控制原发病，保护胃肠粘膜屏障，防治消化道出血及营养支持等）；治疗组加用自拟中药通腑逐瘀汤，每日１ 剂，治疗８ 日。结果：治疗组各项观察指标恢复快，并能有效阻止病情发展，治愈率明显高于对照组（２组分别为６６．７％ 及２７．８％ ，χ２ ＝ １６．３８４，Ｐ＜ ０．００１），病死率明显低于对照组（分别为３１．５％ 及６４．８％ ，χ２＝ １２．０１６，Ｐ＜ ０．００１）。结论：通腑逐瘀汤配合西医治疗新生儿胃肠功能衰竭能有效促进胃肠道粘膜屏障功能的恢复，防止胃肠道有害菌易位及毒素吸收，阻断向多器官功能衰竭的发展，有利于胃肠功能恢复。

Objective:To observe the curative effects of Tongfu Zhuyu Decoction combined with western medicine (WM) on gastrointestinal functional failure(GFF) in infants.Methods:108 ill infants were randomly divided into treated group ( n =54) and control group ( n =54).Both groups were treated with WM,including controlling the primary diseases,protecting the barrier of gastrointestinal mucosa,preventing and treating the hemorrhage of digestive tract,and nutritional supports.In treated group the Tongfu Zhuyu Decoction prepared by authors once a day was added for eight days.Results:The remissions of every observational parameters were rapid,the progressions of pathogenetic conditions were effectively inhibited,and the cured rates in treated group were significantly higher than those in control group (66 7% vs.27 8%,χ 2=16 384, P <0 001),but the mortality rate was significantly lower (31 5% vs.64 8%,χ 2=12 016, P <0 001).Conclusions:The treatment with Tongfu Zhuyu Decoction combined with WM for GFF in infants is able to promote the remisson of barrier function of gastrointestinal tract effectively,to prevent the translocation of harmful bacteria and the absorption of toxins in gastrointestinal tract,impede the progression toward multiple organ failure,and benefit the convalescence of gastrointestinal functions.
