肝海绵状血管瘤的外科综合治疗

Comprehensive surgical treatment for cavernous hemangioma of the liver

目的 探讨多种外科方式治疗肝海绵状血管瘤(LHG)的适应证与手术经验。方法 对我院近10年来经手术切除、瘤体捆扎、肝动脉结扎或栓塞、微波固化术等不同方法治疗的LHG580例进行总结、分析,对手术指征,手术方式选择,术中防止大出血的方法及应急措施,术后辅助治疗等进行讨论。结果 全组术后1个月内死亡2例,占034%;其余均恢复顺利,近5年来已无手术死亡。结论 为提高手术安全性,获得满意疗效,治疗LHG应根据病变大小、部位、全身情况,选择不同的手术方法或联合方式进行综合治疗。

Objective To investigate the surgical indication and effect of various surgical treatment for cavernous hemangioma of the liver (LHG). Methods The clinical data of 580 patients with LHG treated by various surgical measures such as hepatic resection, tumor trussing up, liver arterial ligation/embolism or microwave coagulation of the tumor during the past 10 years in our hospital, as well as surgical indications, selection of operations, management for severe operative bleeding and postoperative supplementary treatment were summarized and assessed. Results Of all patients, 2 died (0.34%) and the rest recovered smoothly; but no death caused by the operation was found in the recent 5 years. Conclusions The choice of operation model or comprehensive treatment should be adopted lying on the lesion size, location and patients'systemic situation to improve the safety of the operation and brings a satisfactory outcome of treatment.
