经腹部与经阴道彩超半环状血流信号对异位妊娠的超早期诊断及意义

Ultra-earlier diagnosis and its significance on abdominal and transvaginal color Doppler flow signals of the semi-ring to ectopic pregnancy

目的探讨经腹部与经阴道彩超半环状血流信号对异位妊娠的超早期诊断价值，以供临床参考。方法将我院2009年1月-2011年3月收治的疑似异位妊娠的89例患者根据孕囊是否破裂，分为破裂组和未破裂组。并选取同期正常子宫妊娠的50例孕妇作为A对照组，选择附件囊肿的47例患者作为B对照组。四组患者均采用经腹部与经阴道彩超进行检查。尤其观察四组患者的半环状血流信号。结果经临床诊断发现，破裂组38例患者中，有22例患者出现半环状血流信号；未破裂组51例中，有35例患者中有半环状血流信号。正常妊娠组50例，31例出现此情况；附件囊肿组仅6例出现此情况。异位妊娠组与正常妊娠组孕妇的半环状血流阻力指数、收缩峰值速度差异不明显，差异无显著性（JD〉0．05）。结论采用阴道超声检查对异位妊娠诊断效果更佳，两者联合使用可提高临床诊断率，而半环状血流信号可作为判断异位妊娠和早期妊娠的临床依据。

Objective To evaluate abdominal and transvaginal ultrasound semi-circular blood flow signals on the uhra-early diagnosis of ectopic pregnancy over the value for clinical reference.Methods 89 patients in our hospital from January 2009 to March 2011 were treated with suspected ectopic pregnancy according to gestational sac broken or not, divided into broken group and non-broken group. And we selected the same period in normal uterine pregnancy, 50 cases of pregnant women as control group A, selected the attachment cyst as the B control group patients （47 cases）. 4 patients were treated by abdominal and transvaginal ultrasound examination. In particular, we observed the four groups of patients with semi-circular blood flow signals. Results Results were statistically analyzed. The clinical diagnosis found that among the 38 patients of rupture group, 22 patients had semi-circular flow signal; 51 patients in unruptured group, 35 patients with semi-circular blood flow signals. Among 50 cases in normal pregnancy group, 31 cases of this situation; attachment cyst group, only 6 cases of this situation. Ectopic pregnancy group of pregnant women with normal pregnancy group semi-circular blood flow resistance index, systolic peak velocity had no significant difference by statistical analysis, P〉 0.05, the difference was not statistically significant. Conclusion The effect of using transvaginal ultrasound diagnosis of ectopic pregnancy is better, their combination can improve the clinical diagnosis, and semi-circular blood flow signals could be used as ectopic pregnancy and early pregnancy clinical basis.
