基于德尔菲法的《伤寒论》医案文献发表质量标准研究

Standard research on the quality of published medical case literature from Treatise on Febrile Diseases based on the Delphi method

目的:建立科学、客观、系统的《伤寒论》医案文献发表质量标准。方法:在前期文献研究的基础上,构建《伤寒论医案文献发表质量评分》条目池,采用改良的德尔菲(Delphi)法,初步编制《伤寒论医案文献发表质量评分》专家问卷,对本市和全国专家进行两轮专家问卷调查,对结果进行统计分析。结果:向本市专家发放问卷8份,外地专家发放问卷31份。所有问卷全部收回,质量合格。经统计分析:"中医辨病辨证依据"、"方药组成、剂量、用法"、"中医诊断标准"、"中医诊断结论"、"描述疗效结果"、"病例病史描述"6项专家认可度较高;"发表期刊杂志等级"、"发表机构级别"、"西医诊断结论"3项专家认可度较低。结论:确定包括"中医辨病辨证依据"、"方药组成、剂量、用法"等在内共计15个条目以评价《伤寒论》医案文献发表质量。

Objective： To establish an objective and systematic standard of the quality of published medical case literature from Treatise on Febrile Diseases.Methods： On the basis of literature search,we constructed the item pool of the report quality of case reports using prescriptions from Treatise on Febrile Diseases.Then,we used the Delphi method and designed a questionnaire named the scoring for report quality of case reports using prescriptions from Treatise on Febrile Diseases.Results： The questionnaire was firstly distributed to eight experts in Tianjin.Then we distributed them to thirty-one experts in the range of the whole country.All of the questionnaires we sent out were replied in high quality.Statistic analysis shows most experts endorse six items in the questionnaire,including the criteria for identifying different diseases and symptoms in TCM;the construction,dosage and the way to take prescriptions;TCM diagnostic criteria;TCM diagnosis;the description of therapeutic effect and the description of disease history.Very few experts accept three items which includes the quality of the journal that a case report is published;the rank of the agency that the head author works for and diagnosis made by western medicine doctor.Conclusion： fifteen items are finally determined to access the report quality of case reports using prescriptions from Treatise on Febrile Diseases.Those fifteen items includes the criteria for identifying different diseases and symptoms in TCM;the construction,dosage and the way to take prescriptions;and so on.
