摘要
目的观察哮喘患者呼出气一氧化氮(FeNO)的表达水平,探讨其与第一秒用力呼气容积占预计值百分比(FEV1%)、外周血嗜酸性粒细胞(EOS)计数、总免疫球蛋白E(IgE)和哮喘控制测试(ACT)评分之间的相关性。方法采用NO分析仪和肺功能仪测定30例发作组、20例缓解组及30例对照组FeNO及FEV1%的水平,同时记录患者ACT评分,并检测发作组中20例外周血EOS计数及总IgE的水平。结果发作组FeNO水平显著高于缓解组及对照组(P<0.01),缓解组也高于对照组(P<0.01);发作组FEV1%的水平显著低于对照组(P<0.01),也低于缓解组(P<0.05);发作组ACT评分低于缓解组(P<0.01)。FeNO的水平与ACT评分呈负相关(r=-0.34,P<0.05),与外周血EOS计数呈正相关(r=0.65,P<0.05)。结论 FeNO的水平能够反映哮喘患者慢性气道炎症,尤其是EOS炎症;并且在评估哮喘控制水平方面可能发挥作用。
Objective To investigate the levels of exhaled nitric oxide in asthmatic patients and its correlation with FEV1%,asthma control test(ACT),eosinophils(EOS) and total IgE.MethodsThe levels of FeNO and FEV1% were measured in 30 asthmatic patients in exacerbation(group A),20 asthmatic patients in stable stage(group B) and 30 normal controls(group C).The ACT score was recorded at the same time.EOS and total IgE were tested in 20 cases of group A as well.Results The levels of FeNO were in an order of group A group B group C(P0.01).The level of FEV1% was lower in group A than that in group B(P0.05) and group C(P0.01).The ACT score was lower in group A than that in group B(P0.01).The level of FeNO was negatively correlated to ACT score(r=-0.34,P0.05),but positively correlated to EOS(r=0.65,P0.05).ConclusionFeNO may serve as a good biomarker for evaluating the airway inflammation,especially EOS inflammation,and its control in asthmatic patients.
