股骨转子间骨折内固定失败后的翻修治疗 被引量：2

摘要 目的 探讨股骨转子间骨折内固定失败后的翻修治疗. 方法 2005年6月至2010年9月共收治20例股骨转子间骨折内固定失败患者,男5例,女15例；年龄47～89岁,平均73.6岁.原始骨折按改良Evans分型：Ⅰ型7例,Ⅱ型6例,Ⅲ型2例,Ⅳ型3例,Ⅴ型2例.以个体化治疗方案,分别采用更换内固定治疗4例,人工关节置换术治疗16例.通过患者术中、术后一般情况、肢体长度、疼痛评分、骨折愈合情况及术后12个月髋关节Harris评分等综合评价其临床疗效.结果 20例患者中2例发生术后并发症,经治疗痊愈,1例术后18个月死亡.术后6个月内固定患者均达到临床愈合,关节置换患者未发现骨量丢失.术后12个月患者疼痛评分平均为（4.5±2.7）分（3 ～7分）,较术前[（7.7±2.1）分]显著降低,差异有统计学意义（t=1.574,P＜0.05）；Harris评分平均为（84.0±14.2）分（65 ～93分）,较术前[（38.0±11.7）分]显著提高,差异有统计学意义（t=3.081,P＜0.05）. 结论 股骨转子问骨折内固定失败后需制定个体化治疗方案,通过更换内固定或行人工关节置换可获得较好的临床疗效.
