缩宫素的三种用药方法对剖宫产患者血液动力学的影响 被引量:3

摘要 目的研究对比剖宫产患者采用静脉推注、静脉缓慢滴注和子宫底肌注缩宫素三种不同的方法对血液动力学的影响。方法选择120例连续硬膜外麻醉下剖宫产的患者随机分成A、B、C三组,待胎儿娩出后立即A组静脉推注缩宫素20 U;B组静脉缓慢滴注乳酸林格式液500 mL加缩宫素20 U;C组子宫底肌注缩宫素20 U。观察三组患者用药后的MAP、HR变化。结果 A组患者的循环功能明显具变化,B组患者的循环功能稍有变化但没有A组明显,C组患者的循环功能无明显变化,A组与B、C两组比较差异有统计学意义(P<0.05或P<0.01)。结论剖宫产术中静脉应用缩宫素对患者的循环功能短时间会产生影响,而静脉推注对循环功能影响更明显。 Objective To study the influence of intravenous bolus injection, slow intravenous infusion and intramuscular injection of oxytoein in the fundus uteri on the hemodynamies of patients receiving cesarean section. Methods All of 120 patients received cesarean section under continuous epidural anesthesia were randomly divided into group A, group B and group C. Immediately after the delivery of baby, group A was given the intravenous bolus injection of 20 U of oxytocin, group B was given the slow intravenous infusion of 500 mL of lactated ringer's solution and 20 U of oxytocin, and group C was given the intramuscular injection of 20 U of oxytoein in the fundus uteri. The MAP and HR changes of the three groups of patients were observed. Results Patients of group A had obvious changes in the circulatory function. Patients of group B had slight changes in the circulatory function,which were not as obvious as those of group A. Patients of group C had no obvious changes in the circulatory function. Group A was significantly different from group B and group C (P 〈0.05 or P 〈 0.01). Conclusion In the cesarean section,the intravenous application of oxytocin has an influence on the circulatory function in a short time while the intravenous bolus injection has a more obvious influence on the circulatory function.
