Manifestation of welder's lung in high resolution CT at the normal chest film

摘要 目的评估胸片表现正常的电焊工肺部高分辨率计算机X射线断层扫描(high resolution computed tomography,HRCT)的异常表现,为早期预防电焊工尘肺提供影像学依据。方法对同一工作环境下的60名30 d内胸部高千伏摄片正常的电焊工人进行HRCT检查,由3名放射科医师分别进行读片,对读片结果采用SPSS 11.5软件进行统计学分析。结果 HRCT可显示电焊工肺部小叶的异常,3名放射科医师读片结果无明显差异(P>0.05)。电焊工肺部异常早期主要表现为小叶中心的磨玻璃结节(16.1%),其次为小叶中心分支状细线影(6.67%),少数(2.78%)为混合型。工龄长的工人较工龄短的更易出现肺小叶内的异常。结论 HRCT是发现电焊工人肺部早期异常的敏感方法,有助于早期干预和预防电焊工尘肺。 Objective To evaluate the abnormal findings of welder' shmg of normal chest film with HR,CT at same working condition, the imaging basis was provided for preventing from the welders pneumoconiosis. Methods Sixty welders were examined with HR CT, whose chest films were normal obtained in 30 days, and the CT films were read 3 radiologists respectively. Statistical analysis was used with SPSS 11.5 software. Results Abnormalities of pulmonary lohule were found in HlZCT. The outcome of 3 radiologists was consistent and had a high degree of correlation. Predominant HRCT findings were poorly-defined ground-glass attenuation centrilobular micronodules (16.1%), branching linear structure 6.67%), mixed type(2.78%).The abnormalities of pulmonary lobule were more fi-equent at longer work age. The results of reading film were dependable and repeatable. Conclusion HRCT is a sensitive method to show early abnormalities of welder' s lung and is helpfill in preventing pneumoconiosis.
